Subject: Form A Review— North St. Extension
Engineering has reviewed the plan titled " Plan of Property in Agawam Massachusetts,Leonard Matz,
41 Belmont Ave.., Feeding Hill, MA., dated March 2006." The plan was prepared by Anderson
Associates, 375 Walnut St. Ext. P.O. Box 382, Agawam, MA 01001.
There are no comments on the proposed layout of the lot.
Additional Comments— For the benefit of the owner and their designer for the anticipated
development of this lot for a single family residential home, I offer the following comments.
The area to be developed appears to be within or near a wetland. A Request for Determination or
Notice of Intent would be required for any construction.
There is a culvert within or near the newly proposed parcel. This parcel conveys flows from the
southern side of North St., under the roadway , and to the wetland area within or near this parcel.
Record plans are available which show the culvert. The culvert is in fair condition and made of
corrugated metal pipe. It is 2.5' to 3' below the roadway surface. The downstream end is"/2 full of
soil/debris. The upstream end is covered with leaves. It may be required to replace this --30 year old
pipe with a new culvert, as part of this project.
