TR-2014-17 A RESOLUTION ACCEPTING A GRANT IN THE AMOUNT OF$71,164.00 FROM THE MASSACHUSETTS EMERGENCY MANAGEMENT AGENCY PURSUANT TO MASSACHUSETTS GENERAL LAWS CHAPTER 44, SEC- TION 53A TO THE TOWN OF AGAWAM Sponsored by Mayor Richard A Cohen WHEREAS, the Town of Agawam applied for and received a grant from the a Massachusetts Emergency Management Agency; and ` WHEREAS,the Town of Agawam desires to accept said grant; and WHEREAS, the grant is in the amount of seventy one thousand one hundred ror- 'x ty six dollars and no cents ($71,164.00); and Y -,, WHEREAS, the Town of Agawam will use the grant funds to purchase and have installed an emergency backup generator for the Town Hail; and -� WHEREAS, it is in the best interests of the Town of Agawam to accept the grant from the Massachusetts Emergency Management Agency. GP funding for the Town of Agawam Town Hall Generator project. The Town of Agawam has received a FF,MA award of$ 71,164.00 and will be reimbursed up to 75% of approved, allowable, and eligible costs, up to the award, as stipulated by the grant agreement and OMB Circular A-87. This is a reimbursable grant program and expenses have to be incurred and paid prior to being reimbursed. Please noie that any project revisions,changes or deviations from the FEMA-approved grant application must be approved in writing by both MEMA and FEMA in order to be eligible for grant reimbursement. In order to execute this agreement., the following tasks relative to the attached grant agreement must be completed; much of the information required can be retrieved from your grant application and/or any supplemental information you have previously submitted. All of the forms associated with this contract are available electronically. Please contact Sherry Leung at 508-820-1436 or e-mail shirletta.lcung@statc.ma.us. 1) Contractor Authorized Sigeatory Listing: The CEO must sign and execute the CASL according to instructions provided. Each signatory must have their signature notarized. 2) Standard Contract Form: The CEO must complete, sigh and HAND date the form as the Contractor. 3) Scope of Work: Approved Scope of Work is included in contract package. 4) Budget Information: The budget has been filled out, according to the approved budget included in your application. Please provide a state fiscal year (July 1 thru June30) budget for the life of the grant for the federaJ funds only. The CEO will sign the budget form. 5) Work Schedule: Please complete work schedule to coincide/fall within the contract start and end dates. Estimate first task start date 90 days from receipt of contract package. 6) Additional Terms and Conditions: These are conditions set forth by MEMA. These conditions/requirements must be satisfied to be eligible for funding. 7) Designation of Program Manager Form: The (T-0 must appoint a locai Projcct - 'Aanager for this Agreement.
