Per your request, we have reviewed the Site Plan entitled"Proposed Residential Development-Jack's Drive at
1004 Springfield Street, Agawam, MA; Parcel ID: F10 6 18; Prepared for. 1004 Springfield Street LLC,P.O.
Box 1242, Southwick, MA 01077; Prepared by: R Levesque Associates, Inc., 40 School Street, Westfield MA
01085; Scale. 1" =20'; Updated: March 28, 2016. R Levesque Associates (RLA) has addressed most of our
comments from our memo dated March 3, 2015. However,and we have the following remaining comments:
.1. The pitch of the proposed road near the intersection with Springfield Street is 5%. The Town Standardd
of 2.5%or less intersection grade for I00 feet.
a. With approval from Engineering, the section of the proposed road from the intersection of
Springfield Street to approximately 50 feet into the project site has been re-graded so that the
pitch of the roast is 2.5%in accordance with Section 159-8.D of the Town Ordinance.
2. A profile view of the proposed road which shows locations and elevations of all new utilities shall be
included in the site plans.
a. The proposed locations of water laterals should be shown on the profile to verify there will be
no conflicts. A separate profile of the proposed drainage system showing sewer and water
utilities should be included as well. RLA has provided Engineering with this information.
3. The proposed sidewalk needs to be extended so it co,rnects units #3 through #6 with the existing
sidewalk on Springfield Street. Also, a cross walk connecting units #2 and #3 shall be included. All
proposed sidewalks shall have a cross slope of no more than 2%and meet ADA standards.
a. The Town of Agawam recently adopted a Complete Street policy which encourages new
construction designs to accommodate all modes of transportation in a context sensitive manger.
Due to limited traffic on this road, engineering recommends, but does not require, the sidewalk
to be extended along the entire length of the proposed road.
4. Due to the high groundwater recorded on the soil test pits, engineering is concerned that there will be
slow infiltration rates within the proposed detention pond particularly during the winter season or heavy
storms events. Engineering has used the HydroCAD files provided by R Levesque Associates to
calculate the I00 yearpeak storm elevation as 202 25 feet if there is no infiltration within the pond. The
rim elevation of the detention pond should be set at 203.25 feet in order to provide a minimum of I foot
of freeboard for this condition_

S. Engineering recommends sufficient separation from the edge of the detention pond to the nearest

pond on the standards within the MassDEP Handbook.
6. The detention pond shall be graded in such a way that the owner will be able to perform routine
maintenance and mowing.

7. The northern proposed catch basin at the site entrance has an outlet pipe that flows in a counter
direction to the stormwater runoff coming down the road. When runoff enters the catch basin, it will be
moving in a southeasterly direction while the water within the catch basin will be moving in a
northwesterly direction as it tries to enter the outlet pipe. ?'his may cause turbulence in the catch basin
and subsequent backing up of the catch basin during extreme events. The northern catch basin should
be relocated so that the outlet pipe is flowing in the same direction as the runoff'

8. Engineering is concerned that the two catch basins proposed at the site entrance will not be able to
capture all of the stormwater runof'from the new road The steep slope of the road may cause the
runoff to}low over the grates of the catch basins and into Springfield Street. An additional catch basin
needs to be installed on the north side of the street at the western edges of the driveway of units#1 & #2,
approximately SS feet from the Springfield Street intersection (see attached sketch).
a. Complete. RLA has supplied Engineering with calculations that confirm the two proposed catch
basins at the entrance of the proposed road will be able to capture all of the stormwater runoff
from the project site generated by the 100-year storm event. Based on these calculations, it is
not necessary to install a third catch basin within the road.
9. The plan should show existing contours at 2 foot intervals extending at least 30 feet beyond the property
lines. Also, the locations of buildings and structures on all abutting parcels should be shown.

10. Per the Deputy Supt. of Water/Sewer, the proposed water valve on Springfield Street shall not be located
within the travel lane. It should be installed behind the sidewalk on the northern side of Springfield
Street. All other proposed water valves on the project site shall be installed outside of the road travel
lanes. If the valves are installed within the road, a double gate valve box will be required.

11- The contractor shall use a long sweep when connecting the building sewer from Units #1 and#2 to the
Springfield Street sewer main.

12.Developer shall inspect the existing sewer lateral from Springfield Street to verify that it does not need
to be repaired or replaced.

14. The section of Springfeld Street from Mill Street to Sarat Ford is under the 5--year mad opening
moratorium until the end of 2016_
a. The Town and the Contractor shall collaborate in order to minimize the area of road opening
required within Springfield Street.
15. Engineering recommends, but does not require that the gas and electricity utilities be installed within
the proposed road
a. The final locations of the electric and gas utilities shall be coordinated with the Town and.utility
companies in the field during construction
16 Several of the proposed planted trees will impact the new utilities on the site (see attached sketch). The
locations of these trees should be revised.
a. 4n Sheet C-5, a proposed tree located at the stairs of Unit 5 appears as if it might conflict with
the gas and electric utilities. It was stated under Comment#15 that the final locations of these
utilities shall be determined during construction. The proposed tree in this location will need to
be planted, far enough away from the utilities so that its mots do not interfere with the services
or future repair work.
17. 7'he existing light pole on SpHni field Street near the proposed site entrance (UP #118165) is currently
25 watts. The light shall be replaced with a 100 watt bulb so as to provide better illumination of the
intersection.
a The Contractor will collaborate with the DPW to replace the light bulb during construction.
18. No parking signs shall be installed on the road and turn around area to keep emergency access open at
all times.

Per your request, on July 7, 2016; Engineering performed an inspection of the site and the following items
remaining to be completed on Jasmine Lane:
1. Building lot bonds in the amount of$3,950.00 per lot are to be set for all uncompleted vacant lots.
There are currently 3 vacant lots: 2, 5 and 7 in the subdivision. Please see section 159-7 H(5) of Town
code.
I The fence around the detention pond needs to be repaired. Locks are to be placed on the gates and keys
supplied to the DPW and the homeowner.
3. The existing stop sign has to comply with MUTCD standards (RI-1 30"by 30"). The reiroreflectivity for
all highway signs is to be high intensity prismatic grade.
4. The developer needs to provide a minimum of 2 shade trees on lots: 6 and 7 are missing one tree. See
attached Town Code.
5. Any remaining debris or materials on site from the construction of the subdivision will need to be
removed and property disposed.
6. Filter fabric shall be removed from outlets and drywell in detention pond.
7. Silt fence and hay bales needs to be remove once vegetation is established nearby on site.
