Associates Surveyors, Inc. on behalf of the Carraige Funeral Holdings, Inc. for the proposed parking
lot addition to be located at the Colonial Forastiere Funeral Home at 985 Main Street in AgawaI)'L
Based on our review, the Engineering Department has the following comments.
1. We have detennined that the parking layout shown in the revised site plan differs from the
actual parking layout of the funeral home. Aerial photography indicates that there is a line of
9 parking spaces On the south side of the parking lot that is not included on the site plan or in
the total parking space count. .
2. The proposed handicapped (HC) parking space needs to be closer to the entrance of the'
funeral home building. Massachusetts regulation 511 CMR 23.3.1 states that He parking.
spaces shall be located on {he shortest accessible route of travel from adjacent parking to an
accessible entrance. It is recommended that the proposed He space be moved north to the
line of 8 spaces directly east from the existing He parking. All HC spaces need a stripped.
access isle no less than 5 feet wide.
3. If the proposed addition requires any cut and/or fill, proposed grades must be included on the
site plan for review.

Per your request, received on June I, 2010, we have reviewed the Applications for Site Plan
Approval Nbmitted by Smith Associates Surveyors, Inc. on behalf of the Carraige Funeral Holdings,
Inc. for the proposed parking lot addition to be located at the Colonial Forastiere Funeral Home at
985 Main Street in Agawam. Based on our review, the Engineering Department has the following
comments.
1. After a site investigation and studying recent aerial photos, we have determined that the
parking layout shown in the site plan differs from the actual parking layout of the funeral
hoxp.l!l. Based on our count, there are 89 existing regular parking spaces and only 3
handicapped (HC) spaces. The parking lot addition will add 31 new spaces, bringing the
total to 120 spaces. Two additional He spaces with access aisles no less than 5 feet in width
must be added in accordance with ADA regulations.
2. A silt fence with hay bales needs to be used to insure that sediment and debris are contained
onsite during construction and are not allowed to flow offsite to receiving waters. The
loclltions of the silt fence and hay bales need to be shown on the site plan. Details should be
included as well.
3. If thll proposed addition requires and cut and/or fill, a grading plan must be submitted to the
En&ineering Department for review.
