On May 4,2011, the Town received a zone change request from Florida towers LLC and Agawam
United Methodist Church to include approximately 15 .17 acres of land located on the south side of
Mill Street in Agawam's Telecommunications Wireless Overlay District (TWOD). The current
underlying zoning is Residence A-2 and Business B.
The applicants are requesting the Zoning Map amendment to enable them to install a
telecommunications tower at the rear of 491 Mill Street. The property is currently occupied by a
single family dwelling and is owned by the Agawam United Methodist Church. The proposed tower
and equipment compound is located 425 ' from the existing hoUse and 41 5' from the church located at
459 Mill Street. The tower is also proposed to be located 376' from the church's playground and
315' from the church's pavilion.
The Planning Office has conducted a review of t1tis proposal and submits this report for your
information.
Land Use and Zoning
As stated above, the property for which the zone change is being requested is currently zoned
Business B and Residence A-2. Please see the attached parcel zoning map and refer to the Agawam
Zoning Map to see the current boundaries of the Overlay District. There are no portions of the
Overlay District in the vicinity of the proposed site.
It was the Town's intent in 1998, when the Telecommunications Overlay District was adopted, that
adequate areas be identified to allow for complete coverage throughout the Town. As such, it is the
• •
responsibility of the applicant to provide evidence by way of service maps to show gaps in current
service levels. The materials as submitted do not provide this type of support data and/or
documentation.
The adjacent land uses are varied. To the west of the site is low density residential development To
the east is a more densely developed residential subdivision. Immediately to the north on the
opposite side of Mill Street is the Agawam High School and to the south is Rt. 57.
Environmental Considerations
A large portion of the 15 acre site is constrained by steep slopes, wetlands and a tributary of Three
Mile Brook. It is difficult to tell by reviewing the submitted plan, but at a minimum, the applicant
should file a Request for Determination of Applicability with the Agawam Conservation
Commission.
Other Considerations
If the application for inclusion into the Overlay District is approved, a Special Permit application
must be filed with the Agawam Board of Appeals and a Site Plan with the Agawam Planning Board.

(the "Church"), we are pleased to submit this petition (the "Petition") to the Agawam City
Council (the "Council") through the Agawam Zoning Officer, Mr. Dominic Urbinati, to
respectfully request the addition of the parcels ofland owned by the Church and located at 459
Mill Street, Agawam, Massachusetts, further identified as Assessor's Map HI 0, Block 2, Parcels
IS, 16 and 17 (the "Site"), into the list of parcels in the Wireless Telecommunications Overlay
District ('"WTOD") as enumerated in Section 180-94 of Article XIV of the Town of Agawam
Zoning Ordinance (the "Ordinance"). We note the Site is split into two zoning districts. The
rear three-quarters of the Site is located in the Business B zoning district. If approved, FTP and
the Church would eventually seek to locate the tower within the Business B zoning district on the
Site. Pursuant to Article IX, Section 180-48, we note that a wide array of business uses are
allowed in the Business B zoning district. Likewise, the rear of the Site abuts a state highway.
The front quarter of the Site is adjacent to Mill Street, contains the church building and is located
in a residential zoning district.
Included herein and attached to this Petition, we have provided the necessary information
including zoning information, general location of the Site, legal description ofthe Site, 38 copies
of plans stamped by a registered land surveyor and the $500 filing fee. We have also included a
letter of support from New Cingular Wireless pes, LLC d/b/a AT&T Mobility ("AT&T"). As
you may know, AT&T is licensed by the FCC to provide personal wireless services throughout
Massachusetts, including the Town of Agawam. FTP anticipates that AT&T will become the
anchor tenant at the Site if ultimately approved. Lastly, we have also included a letter of support
from the Agawam Fire Department which indicates a clear public safety need for the tower and
emergency communications in this area of the Town of Agawam. We respectfully suggest that
the inclusion of the Site into the WTOC will satisfy the purpose as provided in section 180-94 of
the Ordinance noted below. Based 011 our meeting last year, we note a proposed tower would
satisfy all setback requirements under the Ordinance and we have enclosed a preliminary set of
plans for your review.

As will be evidenced at the public hearings associated with this Petition, the use of the
Site for a personal wireless service facility will protect the scenic, historic, natural and other
resources of the Town of Agawam while allowing the provision of personal wireless services.
fTP and AT&T have conducted exhaustive searches in this area of the Town of Agawam to
locate existing tall structures from which personal wireless services could be provided; however,
no such structures were available. FTP and AT&T then searched for parcels in the area at the
required elevation which could provide the necessary access and provide benefits to the
community. In this instance, FTP was able to identify the Site owned by the Church. As noted
above, the rear ofthe Site is located in the Business B zoning district. has the necessary elevation
and access, and will provide the coverage needs for AT&T. The Site is well screened by the
church building, associated facilities and the existing vegetation to the maximum extent possible.
Lastly, the inclusion of the Site into the WTOD will assist the Town of Agawam in complying
with its obligations under the federal Telecommunications Act of 1996.
We understand that the Council will submit this Petition to the Agawam Planning Board
for review. We look forward to presenting the Petition at the public hearings. If there are any

Thank you for contacting the fire department with your willingness to offer tower space and connection
to backup generator power atop of your proposed cellular tower at the United Methodist Church in
Agawam. Upon evaluation of our current communication status, your offer could not have come at a
better time.
As you know, communication failure is the number one cause of catastrophic events on an emergency
scene. In anticipation of a need for redundancy and cross-communications with the police department,
we have been working closely with Sgt. Donovan to formulate a plan to effectively manage these
incidents. Unfortunately, cost constraints have restricted us from implementing such a system. With
your generosity, we will now be able to provide enhanced safety for our firefighters and police officers
as we deal with hostile and dangerous situations on an increasingly frequent basis. The construction of
your cellular tower will allow Agawam's first responders to not only allow emergency responders to
cross-communicate, but also maintain a surety that "communication failure will not be an option" while
protecting the community we proudly serve.
We anxiously look forward to the completion of your project and working with you in order to
implement the expansion and increased coverage of emergency communications to the City of Agawam.

include this property in the Town's Wireless Overlay District.
The Assessors Office lists you as an abutter and therefore must be notified of the upcoming
public hearing. The Planning Board will be holding a public hearing on this request on
Thursday, June 2, 2011 at 7:00 PM. The meeting will be held at the Agawam Public Library,

On behalf of New Cingular Wireless PCS, lLC by and through its manager AT&T
Mobility Corporation ("AT&T"), licensed by the FCC to provide personal wireless services
throughout Massachusetts, including the Town of Agawam, we are writing in support of the
petition by Florida Tower Partners, LLC ("FTP") and the Agawam Methodist Church (the
"Church") to include the above-referenced parcel of land (the "Site'') into the Town of
Agawam'S Wireless Telecommunications Overlay District ("WTODj. As you may know,
AT&T is aggressively eltpanding its coverage area throughout Massachusetts. Consistent with
this objective, a personal wireless service facility at the Site would be of specific interest to
AT&T and provide much needed coverage for AT&T. Based on AT&T's existing significant
gaps in coverage in this area of Agawam, AT&T strongly supports FTP's Petition to add the Site
into the WTOD.

Thank you for contacting the fire department with your willingness to offer tower space and connection
to backup generator power atop of your proposed cellular tower at the United Methodist Church in
Agawam. Upon evaluation of our current communication status, your offer could not have come at a
better time.
As you know, communication failure is the number one cause of catastrophic events on an emergency
scene. In anticipation of a need for redundancy and cross-communications with the police department,
we have been working closely with Sgt. Donovan to formulate a plan to effectively manage these
incidents. Unfortunately, cost constraints have restricted us from implementing such a system. With
your generosity, we will now be able to provide enhanced safety for our firefighters and police officers
as we deal with hostile and dangerous situations on an increasingly frequent basis. The construction of
your cellular tower will allow Agawam's first responders to not only allow emergency responders to
cross-communicate, but also maintain a surety that "communication failure will not be an option" while
protecting the community we proudly serve.
We anxiously look for 1 to the completion of your project and working with you in order to
implement the expan5. __ , and increased coverage of emergency communications to the City of Agawam.

60th anniversary
Harry and Marie (Nason) McCausland, of Feeding
Hills, recently celebrated their 60th wedding anniversary
with a trip to oakRidge, Tenn, to visit with their oldest
son and daughter-in-law, Rev. Randy and Sue
McCausland.
Harry and Marie were married in Ashland, Maine on
Dec. 24, 1950. They have two sons, Randy of oak Ridge,
and Roduey, of Feeding Hills.
They have two granddaughters, Tanya and Rev.
Anthony Cohron, of Jacksonville, F1a., and TJ and Rev.
Luke St CIait,of Anderson, Iod.
They also have one great-grandson, Ethan Cohron, and
four great-granddaughters, Madalynn Cohron, Lauren,
Gracelyn and Jocelyn St. CIait.
Harry retired in 1992 from Feeding Hills Floor service,
which he owned and operated. Marie worked for Wallace
Mfg. for 31 years.
They reside on North Westfield Street in Feeding Hills.

Dear Honorable Members ofthe Town of Agawam City Council:
On behalf of Florida Tower Partners, LLC ("FTP") and the Agawam Methodist Church
(the "Church"), we are pleased to submit this petition (the "Petition") to the Agawam City
Council (the "Council"") through the Agawam Zoning Officer, Mr. Dominic Urbinati, to
respectfully request the addition of the parcel of land owned by the Church and located at 459
Mill Street, Agawam, Massachusetts, further identified as Assessor's Map HID, Block 2, Parcel
15 (the "Site"), into the list of parcels in the Wireless Telecommunications Overlay District
("WTOD") as enumerated in Section 180-94 of Article XIV of the Town of Agawam Zoning
Ordinance (the "Ordinance"). We note the Site is split into two zoning districts. The rear two-
thirds of the Site is located in the Business B zoning district. If approved, FTP and the Church
would eventually seek to locate the tower within the Business B zoning district on the Site.
Pursuant to Article IX, Section 180-48, we note that a wide array of business uses are allowed in
the Business B zoning district. The front third of the Site is adjacent to Mill Street, contains the
church building and is located in a residential zoning district.
Included herein and attached to this Petition, we have provided the necessary information
including zoning information, general location of the Site, legal description of the Site, 38 copies
of plans stamped by a registered land surveyor and the $500 filing fee. We have also included a
letter of support from New Cingular Wireless PCS, LLC d/b/a AT&T Mobility ("AT&T"). As
you may know, AT&T is licensed by the FCC to provide personal wireless services throughout
Massachusetts, including the Town of Agawam. FTP anticipates that AT&T will become the
anchor tenant at the Site ifuItimately approved. Lastly, we have also included a letter of support
from the Agawam Fire Department which indicates a clear public safety need for the tower and
emergency communications in this area of the Town of Agawam. We respectfully suggest that
the inclusion of the Site into the WTOC will satisfy the purpose as provided in section 180-94 of
the Ordinance noted below.
As will be evidenced at the public hearings associated with this Petition, the use of the
Site for a personal wireless service facility will protect the scenic, historic, natural and other

FTP and AT&T have conducted exhaustive searches in this area of the Town of Agawam to
locate existing tall structures from which personal wireless services could be provided; however,
no such structures were available. FTP and AT&T then searched for parcels in the area at the
required elevation which could provide the necessary access and provide benefits to the
community. In this instance, FTP was able to identify the Site owned by the Church. As noted
above, the rear of the Site is located in the Business B zoning district, has the necessary elevation
and access, and will provide the coverage needs for AT&T. The Site is well screened by the
church building, associated facilities and the existing vegetation to the maximum extent possible.
Lastly, the inclusion of the Site into the WTOD will assist the Town of Agawam in complying
with its obligations under the federal Telecommunications Act of 1996.
We understand that the Council will submit this Petition to the Agawam Planning Board
for review. We look forward to presenting the Petition at the public hearings. If there are any

Thank you for contacting the fire department with your willingness to offer tower space and connection
to backup generator power atop of your proposed cellular tower at the United Methodist Church in
Agawam. Upon evaluation of our current communication status, your offer could not have come at a
better time.
As you know, communication failure is the number one cause of catastrophic events on an emergency
scene. In anticipation of a need for redundancy and cross-communications with the police department,
we have been working closely with Sgt. Donovan to formulate a plan to effectively manage these
incidents. Unfortunately, cost constraints have restricted us from implementing such a system. With
your generosity, we will now be able to provide enhanced safety for our firefighters and pollee officers
as we deal with hostile and dangerous situations on an increasingly frequent basis. The construction of
your cellular tower will allow Agawam's first responders to not only allow emergency responders to
cross-communicate, but also maintain a surety that "communication failure will not be an option" while
protecting the community we proudly serve.
We anxiously look forward to the completion of your project and working with you in order to
implement the expansion and increased coverage of emergency communications to the City of Agawam.

GRADING & EXCAVATING NOTES:

MINIMIZE SEDIMENT LEAVING THE SITE.
2. PRIOR TO COMMENCING LAND DISTURBANCE ACTIVITY, THE
LIMITS OF LAND DISTURBANCE SHALL BE CLEARLY AND
ACCURATELY DEMARCATED WITH STAKES, RIBBONS, OR
OTHER APPROPRIATE MEANS.

2. WHEN USING VALMONT COMPONENTS FOR SPLICING BRIDGE CHANNEL SECTIONS, THE SPLICE SHOULD
