At its duly called meeting held on March 15,2012, the Agawam Planning Board voted to
approve the Site Plan for 1775 Main Street with the following conditions: the
Engineering Department comments dated March 5, 2012 (attached) with the exception of
#1 shall be addressed; the following note shall be placed on the plan "All plantings must
be maintained and that portion of the public way abutting the parcel and including the
treebelt shall be maintained in a safe and attractive condition by the owner of the parcel.";
and there shall be no dumpster on the site. Three sets revised plans are to be submitted to
the Planning Board.

Per your t<lq\lC3t, we have reviewed the plan for Massachusetts Veterinary Cardiology SQviccs, Inc. (Nancy
Morris, DVM); 1775 Main St, Agawam, MA and we have the folIowing comments:
1. The plans should be drawn to scale and be stamped by a Professional Engineer, Land SurveyOf or
Landscape Architect unless as specified by the Planning Board.
2. The accessible parldng and ra..;np shall meet all applicable Federal, State and Town Standards (see
attached site measurements and details).
3. The parking spaces along the southern property line will need to be removed as there is not sufficient
area for the turning movements of vehicles.
4, All parking spaces shall meet the minimum size standard.
5. A dumpste. (if necessary) will need to be fenced in.
6. It is recommended !hat curb SlOpS be placed at the ends of the parking stalls neat the building to stop
vehicles from hitting the building or overhanging the walkway.
7. The applicant sha1l walk with the Engineering Division if a business sign is proposed in any location or
height other than the existing sign. It is important that the new sign will not impede sight distanCe for
vehicles exiting the site or Woodcliff Ave.
8. Nothing shall be placed on the north east comer of the property that would impede sight distance at the
intersection of Woodcliff Ave. IUld Ruskin St.

veterinary cardiology office.
Description of Site: Site is currently fully developed. The only change being
proposed is the creation of a small fenced in area at the rear of the building.

Provision for Traffic Flow: The applicant does not propose to change the existing
curb cuts. The site has a 30' wide curbcut on to Main Street and a 29' wide curb cut on
to Woodcliff Ave.
Parking: The plan shows seven (7) parking spaces in the front of the building.
Employee parking would be provided at the rear of the building. The parking as shown
on the plan at the front of the building is not useable. Please see Engineering and the
Safety Officer's comments concerning the parking. The applicant indicated at a team
meeting that she has three (3) employees and not more than two patients at a time.
Drainage: Unchanged.
Public Utilities: See Engineering comments.
Landscaping/Screening and Buffers: The following landscape note must be placed
on the plan: "All plantings must be maintained and that portion of the public way
abutting the parcel and including the treebelt shall be maintained in a safe and
attractive condition by the owner of the parcel." All vegetation obstructing the line of
site along Main Street should be removed.
Sign Location: Applicant indicates her sign will be in the location of the current sign.
She has been advised that she must get a sign permit from the Building Inspector.
Exterior Lighting: Not shown. All exterior lighting must be recessed and shielded.
Wall packs are discouraged. If exterior lighting is to be used location, cut sheets and
specifications must be submitted.
Rendering or Elevations: O.K.
Dumpster Location: Applicant is uncertain if they will use a dumpster. If a dumpster
is used, it must be fenced, gated and screened.
