At its duly called meeting held on November 16, 2012 the Agawam Planning Board
voted to approve the Site Plan entitled "Mass Veterinary Cardiology Services, Inc., 305
Suffield Street, Agawam, MA"; prepared for: Nancy Morris, DVM -P.O. Box 1021,
West Springfield, MA; dated 111112012 with the following conditions:
• the dumpster shall be fenced and screened;
• no additionaIlandscaping is be installed that would impede traffic or
pedestrian safety;
• the handicapped accessible parking signs are to conform to the design and
verbiage specified in MOL C.40 S.22A and AAB 521 CMR 23:00 (see
attached details).
Three sets of the revised plan shall be submitted to the Board for their signatures. If you

Per your request, we have reviewed the site plan entitled "Mass Veterinary Cardiology Services Inc., 305
Suffield Street, Agawam, MA; Prepared for: Nancy Monis, DVM -P.O Box 1021 West Spriogfield, MA;
Scale: NJA; Date: 111112012," and we have the following comments:
I. The accessible parking signs are to conform to the design and verbiage specified in M.O.L. C.40 S.22A
and AAB 521 CMR 23.00. See attached details.
2. Proposed dumpster area will need to be fenced in.
3. The parking lot is currently gravel. At a minimum 50' adjacent to Suffield Street should be paved to
reduce the tracking of lIIl1Ierial into the roadway.
Engineering reserves the right to make additional comments based on revisions made from the comments above
and as new information is submitted. If you have any questions please do not hesitate to contact this division.

The remodeling of this business into the Mass Vet Cardiology Services will have no
impact on pedestrian or traffic safety. Visibility exiting the parking lot is good in both
directions (north and south).
However, the parking lot of this business is not paved and according to the site plan has
12 parking spaces and 1 Handicapped parking space. The Handicapped Parking space
must be in compliance with the Town's Handicapped Parking Ordinance.
