Staff of the Massachusetts Historical Commission (MHC) have reviewed the technical archaeological report
for the intensive (Iocational) archaeological survey, submitted by Gray & Pape, Inc., received January 21,
2015, for the project referenced above.
Results of the intensive (Iocational) archaeological survey for the revised project location yielded no historic
or archaeological resources. Since no significant historic or archaeological resources were identified within
the project area MHC concurs with the management recommendation provided in this technical report. No
further survey is recommended for the project, as proposed. After review of MHC files and the information
provided, MHC has determined that the project, as proposed, is unlikely to affect significant historic or
archaeological resources.
These comments are provided to assist in compliance with Section 106 of the National Historic Preservation
Act of 1966, as amended (36 CFR 800), Massachusetts General Laws Chapter 9, Section 26-27C (950 CMR
70) and the terms of the FCC Programmatic Agreement. If you have questions, please contact Jonathan K.

shown on plan. The existing curb cut will be reused by VZW to access the site, no new curb cut is being
proposed
Parking: Parking for one vehicle located at equipment shelter. We hove added a parking space to the
plans located next to the proposed compound, (Sheet A-I)

that will be reused on the project, we have also called for temporary seeding or silt socks to be utilized

3. Engineering is concerned about fire suppression on the project site due to the proposed propane tank.
There is a hydrant located on Mill Street; however, it is approximately 500 feet away from the compound
area. It is recommended that Verizon-contact the Fire Department to determine If any additional hydrants
or other fire suppression equipment would be required. On Wednesday. January 21, 2015, I meet with
Agawam Fire Inspector Jacob Dushane to review the project and the need for any additional fire hydrants
or fire suppression systems on the proposed project. I my review of the project the fire inspector did not
need an additional fire hydrant and any additional fire suppression system in not needed within the VZW
Equipment Shelter.
4. The orange construction fencing that indicates the limit of work on the plans extend. J 00 feet or more east
of the propo.ed compound area. Will there be more work proposed in this area or will It be used for
stockpile/equipment storage during construction? Silt socks need to be in.taned around ony additional
work areas. The orange construction fence will be installed prior to the start of construction to identify the
limits on the 200' river front area, there will be no work and there will be no access to this area by VZW's
contractor
5. On Sheet C-2, Uncoln Street in mislabeled as Une Street. We have relabeled the street to Lincoln Street
ISheets C-I and C-2)
6. There is an existing catch basin located on Mill Street at the beginning of the proposed gravel driveway.
Engineering requests that snt socks be installed in the catch basin to prevent sediment form entering the
tawn drainage system. We have noted on the drawings that silt socks be added to this catch basin for the
duration of construction and until the areas are stabilized ISheet C-2)
7. Engineering has determined that this project wiO cause no significant increase in impervious area.
Therefore, drainage calculations will not be required by DPW. No comment needed

Dear Ms. Walsh:
Staff of the Massachusetts Historical Commission, office of the State Historic Preservation Officer and State
Archaeologist, have reviewed the FCC Form 620, received November 7, 2014, submitted for the project
referenced above. The proposed project consists of the construction ofa lOS-foot monopole
telecommunications antenna and associated electrical lines, access roadway and support infrastructure within
a 100 by 100 foot fenced enclosure at 459 Mill Street in Agawam.
The MHC has previously reviewed an FCC form 620 for a telecommunication antenna at this property in
20 II that included a preliminary archaeological assessment conducted by Heritage Consultants, LLC. At that
time the MHC indicated that the preliminary archaeological aSsessment.did not consider previous relevant
research results and requested that an intensive (locational) archaeological survey (950 CMR 70) be
conducted for that project.
The preliminary archaeological assessment completed for the currently proposed project was also completed
by Heritage Consultants, LLC, and also does not include review of pertinent previous research results. The
results of reconnaissance archaeological survey conducted for Agawam (MHC# 25-200) included iii the
MHC's Inventory of Historic and Archaeological Assets of the Commonwealth identified the general area
along Mill Street as a location of avocational archaeological collection of ancient Native American artifacts.
Reconnaissance and intensive survey completed for the Route 57 right-of-way (MHC # 25-166, 200)
adjacent to the project area recovered a quartz biface in fill deposits originating from the vicinity of Old Mill
Road, immediately east of the project area in a similar environmental setting. Undisturbed portions ofthe
project area are archaeoJogically sensitive. The archaeological sensitivity of the project area is principally
defmed by its environmental characteristics, which include level, well-drained, elevated soils in close
proximity to the feeder streams of Silver Lake and the Mill Brook, favorable for ancient and historic period
occupation and land use.
The MHC requests that an intensive (Iocational) archaeological survey (950 CMR 70) be conducted for the
project by a qualified archaeological consultant with regional experience in this region of New England. The
purpose of the survey is to locate and identify any significant archaeological resources that may be affected
by the project.

A State Archaeologist's field investigation permit (950 CMR 70) for the archaeological Principal
Investigator is required to ensure that the proposed survey and reporting will meet professional standards.
The results of the survey will provide information to assist in consultation to avoid, minimize or mitigate any
adverse effects to significant archaeological resources. '
The MHC notes that project information has been submitted to the Agawam Historical Commission (AHC)
for the project. Copies of any written comments received from the AHC should be submitted to the MHC.
These comments are provided to assist in compliance with Section 106 of the National Historic Preservation

REASON: Verizon Wireless is seeking a Special Permit in accordance with Section 180-96 of
the Town of Agawam's Zoning Ordinances which would allow for the construction of a
telecommunications wireless service facility at the premises identified as 491 Mill Street and is
also seeking a Variance from Section 180-97 ofthe Town of Agawam's Zoning Ordinances
which would allow for the construction of a telecommunications tower with less than the
required distance from the base of the tower to the property line at the premises identified as 491
Mill Street.
Attached please find 10 copies of the site plan for the above referenced property. These plans are
for your review and comments, and any recommendations you may have so we can consider
these recommendations and comments in our final decision. If you do not respond to this
Board within thirty-five (35) days of the above referenced date, we wiD assume that you
