Agawam Axe House has opened in Western Massachusetts
and they're ready to make the trendy recreational activity a
favorite for everyone.
After visiting an axe -throwing house during a trip to
Canada, co-owner Anneliese Townsend got the idea to
bring it to Agawam, a place where she has familial roots.
"We were in Montreal and decided to try it outs" said
Townsend.

After a year of ideas, Townsend and her co-owner decided

There are currently no age requirements or restrictions at
Agawam Axe House, in fact they plan to host birthday
parties for all ages.
"Agawam Axe House is for anybody who is looking for an
adventure, stress relief, or team building," said Townsend.
"Everyone here is having fun, in a safe environment, that's
what this is about. if
Axe -throwing coaches provide safety protocol prior to

RE: Agawam Axe House Site Plan Review 396 Main St., Agawam, Ma
Dear Planning Board and Ms. Townsend,
Gary Turnbull
Electrical Inspector
Michael Theroux
Sealer of Weights & Measures
Enforcement Officer
I have reviewed the site plan application for the Agawam Axe House submitted to this office on
February 15, 2018. 1 have no objection to its approval since the proposed project involves only
interior work as long as they have a policy regarding age restrictions and no alcohol is permitted
on site.
The Agawam Axe House was sent to the Planning Board for a site plan review pursuant to Town
of Agawam Zoning BylawsChapter 180 Section 13 B since it involves a change in use
" A site plan shall be required and shall be submitted in each of the following situations: (1) Any
proposed residential, business, industrial„ intuitional or other use of a new or renovated sttucture
or of a parcel of land, or an than a in such use structure or parcel, except for one -family
detached dwellings and duplexes on separate lots.",.
I have attached a copy of my cease and desist order issued on January 29, 2018. Since the order
was issued a Team meeting was held on February 9, 2018 at the Agawam Axe House located at
396 Main Street so that applicable Departments could view the operations and address any
concerns. Immediately after the site review, the meeting continued at the Department of Public
Works meeting room. The applicant presented their case and each applicable Department was
able to comment on the project.
In summation it was concluded that no special permit would be required from the Zoning Board
of Appeals for them to operate. There are no regulations in MA about this type of business and
there was no opposition voiced at the Team meeting. It should be noted that the overriding
concern was that there are no age restrictions and of the potential use of alcohol on the premises
has been publicized.
On February 9, 2018 I called Anneliese Townsend (owner of Agawam Axe House) to inform her
that I was withdrawing my order to apply for a special permit with the Zoning Board of Appeals.
Please see excerpt from letter "The business is located in the Business A zoning district which
permits pursuant to Town of Agawam Zoning Bylaw 180-4413 "The major use of a building or
structure in this district shall be to store and display goods for sale at retail on the premises or to
furnish aservice intended for residents of the immediate vicinity. Banks, hotels, dining
establishments, hails and office buildings shall be considered as complying with such
requirements." An axe throwing business would not be permitted in the Business A district
unless approved by the Zoning Board of Appeals." My decision to send to ZBA was based upon
life safety concerns whicharethe basis for our Zoning Ordinance and the Massachusetts State
Building Code which I enforce.
In conclusion, the applicant has submitted all the required information outlined in my cease and
desist letter with the only requirement leftbeing, site plan approval.

Dear Mr. Strange,
aAs the Health Agent for the Town of Agawam, I have reviewed the information provided at the
recent Department Head Team Meeting pertaining to the referenced commercial entity. The
owner must complete a Food Establishment Permit Application and submit that document with
the required fee of $75.00 to the Agawam Health Department prior to selling food to the public.
It is also recommended that the onsite staff update their First Aid Training Certificateswithin;a
reasonable time frame as classes become available.
Provided that these two items are <addressed, the proposed plan satisfactorily meets the basic
principles of protecting public health and community sanitation.

AGAWAM AXE HOUSE
What is Axe Throwing?
Axe throwing is a lot like the sport of bowling or darts, Individuals take turns to throw
an axe at a target which is 12-16 from them, in specified lanes.
The sport is extremely safe sport, and is growing world-wide, 3 Axe Throwing
companies have already announced their plans open in 2018. (Incomplete list of
current US axe throwing locations below).
The Agawam Axe House contains 8 lanes of throwing. These lanes are divided in to
"Pits", each pit contain 2 lanes and each pit is separated by a 10' high chain link fence.
• We have strict regulations for throwing guests, and for spectators, each guest is
given a safety demonstration by our Coaches prior to axe throwing.
• The safety riles are laid out, quite large in every pit and throwing guests can
read them the entire time they are throwing. There are also laminated safety
rules at every lane.
• The entire arena is always supervised by ou-r coaches during any throwing
activity.
• There are clearly defined throwing areas, and spectators areas, and liability
waivers are signed prior to participating.
• We adhere to the regulations set out by the World Axe Throwing League and
have been a member since December 2017. These regulations and distances
were developed to ensure thrower safety during participation.
• The Agawam Axe House offers private parties, corporate team building events,
along with leagues.
e The Agawam Axe House has a General Liability insurance policyto the effect oft
million, which includes million per incident.
The Agawam Axe House replaced the space that was The Range and Training
Academy, where we would regularly hold training classes. Our classroom consisted of
16 seats, and we ran those classes on Weeknights and Weekends. We would have
anywhere from 10-16 participants and up to 3 instructors, while the Gun Store was
open. We previously had a firearm simulator in the space for training and
entertainment purposes which has since been removed and is not going to offered in
the Axe House, The Gun Store is now no longer a retail business. it is simply a
consulting business.

Check in process
Upon entering the Agawam Axe House guests are located in an enclosed Lobby area
with a viewing window, which allows them to determine if they want to participate.
• At that point we confirm reservations and address anyone's safety concerns.
• Each individual (throwing, or not) is required to read, understand and sign our
Iiabilitywaiver (attached).
• No rnember of the public can enter the Axe Throwing arena until the waiver is
signed and they are escorted by a Coach.
• Once all the waivers are signed they are informed that beyond the following
doors there are axes being thrown and that they should always be facing the
"Pits", even when standing in the spectator area.
• They are then assigned their Coach, and that Coach takes the group back to
their assigned lane and begins the safety demonstration.
• The safety rules (attached) are discussed with the entire group, they are also
located in plain sight on the walls of the pits, and also on the tables in the
spectator area.
• The Coach then instructs the group, and one -at -a -time, assists every member of
the group with proper throwing technique.
• After the group is following all the rules, the Coach steps aside and watches
their groups) from within the arena.
• The Coach continues to monitor their group(s) during the duration of their
session and steps in inthe event of unsafe activity,.or guest difficulty.
Coaches can be assigned more than 1 group. We typically will only do this is the event
the 2nd group is approximately 20-30 minutes behind the initial group., it is then the
responsibility of that Coach to monitor both groups, which we tend to assign lanes
close to one another, more than likely in the same pit_
There is at least 1 Coach monitoring activity in the Arena at all times. This is a
requirement by our insurance provider, and we adhere to this very strictly

We have a Standard Operating Procedure for incidents, and each employee is aware of
the location of our first aid kit, and can call 911 in an emergency. We are currently
looming into defibrillator for the facility.

AGAWAM AXE HOUSE
What is Axe Throwing?
Axe throwing is a lot like the sport of bowling or darts. Individuals take turns to throw
an axe at a target which is 12-16' from -them, in specified lanes.
The sport is extremely safe sport, and is growing world-wide, 3 Axe Throwing
companies have alreadyannounced their plans to open in 2018. (Incomplete list of
current US axe throwing locations below).
The Agawam Axe House contains lanes of throwing. These lanes are divided into
"Pits"; each pit contain 2lanes and each pit is separated by a 10' high chain link fence.
• We have strict regulations for throwing guests, and for spectators, each guest is
given a safety demonstration} by our Coaches prior to axe throwing.
• The safety rules are laid out, quite large in every pit and throwing guests can
read them the entire time they are throwing. There are also laminated safety
rules at every lane.
• The entire arena is always supervised by our coaches during any throwing
activity.
• There are clearly defined throwing areas, and spectators areas, and liability
waivers are signed prior to participating.
• We adhere to the regulations set out by the World Axe Throwing League and
have been a member since December 2017. These regulations and distances
were developed to ensure thrower safety during participation.
• The Agawam Axe House offers private parties, corporate team building -events,
along with leagues.
• The Agawam Axe House has a General Liability insurance policy to the effect of 2
million, which included million per incident.
The Agawam Axe House replaced the space that was The Range and Training
Academy, where we would regularly hold training classes. Our classroom consisted of
16 seats, and we rare those classes on Weeknights and Weekends. We would have
anywhere from 10-16 participants and up to 3 instructors, while the Gun Store was
open. We previously had a firearm simulator in the space for training and
entertainment purposes which has since been removed and is not going to offered in
the Axe House. The Gun Store is now no longer a retail business. It is simply a
consulting lousiness.

Check in process
Upon entering the Agawam Axe House guests are located in an enclosed Lobby area
with a viewing window, which allows them to determine if they want to participate.
• At that point we confirm reservations and address anyone's safety: concerns.
• Each individual (throwing, or not) is required to read, understand and sign our
Liability waiver (attached).
• No member of the public can enter the Axe Throwing arena until the waiver is
signed and they are escortedby a Coach.
• Once all the waivers are signed they are informed that beyond the; following
doors there are axes being thrown and that they should always be facing the
"Pits", even when standing in the spectator area.
• After the group is following all the rules, the Coach steps aside and watches
their group(s) from within the arena.
• The Coach continues to monitor their group(s) during the duration of their
session and steps in in the event of unsafe activity, or guest difficulty.
Coaches can be assigned more than 1 group. We typically will only do this is the event
the 2nd group is approximately 20-30 minutes behind the initial group. It is then the
responsibility of that Coach to monitor both groups, which we tend to assign lanes
close to one another, more than likely in the same pit.
There is at least 1 Coach monitoring activity in the Arena at all times. This is a
requirement by our insurance provider, and we adhere to this very strictly.

ESCAPE 413
Summary Overview
Proprietors:
Adam Rovithis (Agawam, MA)
Jared Hamre (Agawam, MA)
Overview:
Proprietors are requesting permission from the Town of Agawam to open an Escape Room
entertainment facility in town. This document will define what an escape room is and identify the
customer experience and safety measures in place.
What is an "Escape Room":
The term "Escape Room" is the general marketing term used to describe a live physical adventure
game in which players solve a series of puzzles and riddles using clues, hints and strategy to
complete the objectives at hand. Players are given a set time limit, typically 60 minutes, to unveil the
secret, which is hidden within the themed rooms. Escape rooms are often also called puzzle rooms.
Some examples of a theme might be a room with a plot surrounding Superhero's Wizard's or a
Doctor's Office.
Why Agawam?
Both proprietors are lifelong residents of Agawam, MA and wish to bring additional entertainment to
their hometown.
Target Market:
All ages are welcome however the recommended age is 13+, simply due to the level of critical`
thinking and problem solving required. At Escape 413, all rooms will be kid -friendly, and there will be
a specialty room specifically for young children with clues based on elementary grade problem
solving. Escape 413 plans to cater to corporate events,' birthday parties and group outings.
lnitial Build:
Escape 413 is looking for approximately 1,200 square feet which would include three themed rooms,
a lobby, and a control room for the game masters.
I
DIMS-1
Are players actually locked in a room?
No. Never.. While some escape room companies do physically lock their players in, we have chosen
to always leave the door unlocked, in case of emergency. Our rooms are designed to be solved or
"won" by escaping with the final clue/item in hand, vs physically escaping by opening the locked door.
For example, a player may win by escaping with a prized diamond from a safe.
Is an Escape 413 employee able to watch the players in case they need help?
Yes. Every group is assigned their own employee which is referred to as their "game master". The
game master will physically be on site but in a separate control room and uses surveillance cameras
with two way audio and the in -room television to interact with the players both visually and audibly. If
players get stuck, they can ask for clue by flipping a designated switchwhichlights a red bulb in the
control room where the game master resides. Players can also simply request a clue out -loud as well.
Are safety instructions given?
Yes. Each room is equipped with a television monitor that allows the game master to interact with the
players and provide clues. Before the game is started, all participants will watch an instructional
video with safety instructions and then be told the story of the room before the timer starts.
How do Mayers know what not to touch?
Actually, the entire room is in play, as players are encouraged to interact with all objects within a
room. Of course, games are designed with this in mind, knowing players are asked to investigate all
items. Any large items or potential safety hazards will be secured safely to a wall or the ground. In
the rare instance when an item needs to remain exposed but is not part of the game play (electrical,
plumbing, exit sign), these items are marked with a bright yellow Happy Face sticker which indicates
that item only is specifically off-limits and is not part of the game. This is reiterated during the
instructional video.
How long are players in the room for?'
Players are given 60 minutes to solve the puzzle. If they do not solve the puzzle in 60 minutes, they
will be greeted by their game master when time is up for the final solutions to be revealed, or be given
an opportunity to re -book the room. If players solve the final clue prior to the 60 minute limit, 'their
game will end when the last clue is .solved..A leaderboard is kept to score the top teams.
How many players are allowed in a room at once?
Each puzzle room is designed with the logic required to be solved by up to players within 60
minutes at a 33% success rate. Therefore, our online booking agent will allow a maximum of
players. The more players in a 'room, the quicker the puzzle is likely to be solved. For larger groups
that wish to play in one room, we will allow up to 10 players in a roam on an exception basis only.
Will ypur rooms be yyheelchair,accessible?
Yes. However, some rooms may require at least 1 player to crawl, which will be disclosed at the time
of booking.
Do you have an example of what the room might look like?
While our final design themes are still being developed, we thought we would include;a photo from an
existingescape room as a representation of what a room might look like:

Below is a basic drawing of plans for "build out' Each room having a different theme, but all following
similar criteria. Each room will have a secondary room that guests will enter with clues given. Walls to

ceiling material as well. Minor Electrical work to be done by licensed electrician as needed.
