The plan proposes significant infrastructure located within an area that is of concern to the DPW. Review of the original sewer construction plan and discussion with DPW personnel found that during construction (-- 1975),there was significant erosion of a large area, the sewer line blew out, the pipe was clamped, and a significant depth of fill was placed to complete the sewer repair. Approximately two years ago, significant erosion of the slope and drainage spill way occurred. The sewer main was exposed. DPW had to implement extensive restoration of the area. This area has had major stability problems two times in the past. The Witheridge St. extension submittal proposes creation of many additional structures in this same area. New roadway, retaining wall, water main extention, new drainage outlet structues, and the placement of 2.5'to 3'additional fill are proposed for an area which has been unstable in the past and has ground water seeping out of the existing slope. The DPW does not want to take future responsibility to repair and replace the public way structures proposed that are anticipated to fail in the future due to soil stability problems. 2. The DPW does not oppose the use of a hammerhead shaped turn-around on a dead end street, but in the proposed location, due to past problems with soil stability, is not appropriate. 3. The current proposal does not show any detention for the proposed drainage system. Detention is required. 4. The current location of rip rap dishes, swale, and level spreader is not appropriate because they are on the side of a slope, and because of soil stability problems in this area in the past and anticipated in the future. The preliminary proposal does not show any stormwater detention system for the changes to drainage characteristics due to the project. This must be shown conceptually. Approval of a conceptual design for a detention system does not necessarily mean that the final design will be feasible. Detention' which is acceptable to the DPW will be required to comply with Town Code and the DEP Stormwater Management Policy for the anticipated Notice of Intent submittal. 2. Previous comments pertaining to the future definitive submittal still apply. 3. The definitive submittal design for the retaining wall must include a stamped geotechnical and structural design to provide long term stabilization of the wall and outlet structure and permanent measures to ensure groundwater does not adversely impact these structures. 4. Any geotechnical or soil investigations for the detention area, the foundation of the retaining wall, and outlet structure must be scheduled such that Engineering may observe. The elevation of the average annual high ground water elevation must be determined by a soil scientist or appropriate geotechnical professional for both the retaining wall,outlet structure location, and the location of any proposed detention structures. Bean Inc., 40 School St., Westfield, MA 01085". The following must be addressed during the-Conservation Committee submital: 1. Proposal does not address pre/post construction impervious areas and the impact on the wetland within the project area. The plan needs to have all pertinent information added to it so that Engineering may perform a storm drainage analysis. Show pre and post upstream drainage scheme to exemplify total extent of storm water impact on the proposed catch basin and wetlands. Also add proposed spot elevations in the turn-around. The following must be addressed during the Preliminary Subdivision submittal: 1. An area is proposed to be built up to provide roadway and frontage at southwesterly corner of proposed `hammerhead' turn-around. The proposed grades without any retaining wall or other means of permanent stabilization is not appropriate. With drainage runoff being directed to this built up area, it is anticipated that the roadway, catch basin, and drainage outlet will become unstable over time. The Town would then be responsible for repairs of the proposed right of way and drainage structures. If a retaining wall is proposed for permanent stabilization, the proposal must include drainage structures to relieve groundwater from the hill, Any drainage outlet at this location must be ended with a concrete head wall which is tied into the slope, so that it does not become unattached and pull the drainage pipe/ and/or catch basin and roadway structures to bottom of slope in the future. Because part of the proposed construction is within this area of very steep embankments, the plan should include details regarding temporary and permanent bank stabilization. Heavier storm flows may require that the rip-rap be PV 0 0 extended beyond the proposed 10 ft. diameter rip-rap dish and to the wetlands rather than use a grass swale as proposed. 2. As mentioned in item##1 above, the proposed construction at the southwest corner of the turn-around has many critical concerns. The specification for the Town's maximum allowable slope is 3:1. The proposed slope in this area is greater than 3:1. The addition of a retaining wall here should include proposed grading contours which are less than 3:1. This should help to soften the impact of storm waters affecting the area. Indicate the overall flow path of storm waters with these grading contours. 3. To facilitate access by city plows and trash trucks, we recommend utilizing the No Parking Any Time traffic regulation by installing the appropriate signage throughout the turn-around area. Also, guardrails in the turn-around will be required to prevent vehicles from driving over the proposed cape cod berm and down the embankment. 4. Show conceptual building foot print locations. Additional items may be required based upon new information submitted in the future. The follQwing must be addressed during the Definitive Subdivision submittal: l. There is one tree outside of the proposed construction area which is leaning dangerously towards the proposed catch basin in the turn-around. This tree will have to be removed before any construction can begin. Care must be exercised in its removal since it is situated in a steep bank next to the wetlands. 2. The locations and details for the erosion prevention measures should be added to the plan. Temporary storm measures above the standard erosion controls will have to be proposed. The construction site is the natural point of exit for storm waters coursing down Witheridge St. Some type of temporary diversion dikes, rip-rap swales, etc. with corresponding details will have to be included on the plan to handle any given storm event until all construction is completed. Show upstream drainage scheme to exemplify total extent of storm water impact on the proposed catch basin. Add proposed spot elevations in the turn-around. 3. Include rim elevation of the existing sanitary manhole in the proposed turn-around. The top of this structure may have to be adjusted. There have been problems with the sanitary sewer system in this area in the past. Care should be exercised during construction so that the least amount of disturbance to the sanitary sewer will take place. 5. Show type and size of the existing and proposed water main. Include water and sanitary sewer laterals on plan. Add note "Sanitary sewer to have minimum cover of 4 feet. Water to have minimum cover of 5 feet." 6. A new water gate valve may be required to be installed at the juncture of the existing and proposed water main to facilitate pressure testing and chlorination of the new water main. PV • 0 7. Indicate the size and type of proposed drainage pipe (with concrete head wall) connected to the proposed catch basin. 8. Roadway, structures, utilities, trenching, head wall, etc. details should be added to the plan. Show saw cut pavement detail. Include all proposed contours where required. 9. Because of the existing grading, some of the storm waters on Witheridge St. travel into the yard of#115 Witheridge St. The homeowner of that residence, Krista Houston asked if berm near her home and regrading along her driveway could be constructed to divert those waters away from her home. The parcel is zoned Agriculture, and the soils shown on the Soil Conservation Service Maps indicate Merrimac soils are present on site. The properties surrounding the site all have residential homes. Municipal Water and Sewer are available to the site, and natural gas exists on Witheridge Street. There is no indication of historic nor archaeological areas with this site. Proposed Dey lloopment: This proposal shows the development of 1.6 acres into 2 building lots, with frontage on a "Tee" type turnaround. Both lots meet or exceed the minimum zoning requirements, and existing municipal services will be utilized for this project. The proposed storm drain system will be designed to collect the runoff through a drop type catch basin at one corner of the turnaround and discharge runoff into a Rip-Rap dish upgradient of a wetland area.
