We found - in the plans that were submitted to us for review time date stamp "2006 Feb. - 3 Al 1:34," that their are no Van Assessable Handicap Parking, and that the current plans could be reconfigured to come in compliance without loosing any parking. Also, we are asking is their a fence gate opening that a wheelchairlha6dicap person have to maneuver? What are the dimensions of the ramp, and is the top of the ramp flat and maneuverable for wheelchair/handicap person? These questions would also effect the recommendations from the Commission. We recommend that the wheelchair handicap parking to include Van Parking, and should be a minimum of 3 handicap parking spaces, in accordance to Local, State, and Federal Codes. Leonard House— 663 Main St. Per the Planning Board's request, I have reviewed the site plan for the proposed building addition to the Capt. C. Leonard House. I offer the following comments. 1. Any work within the Main St. layout will require a permit from Mass Highway. It.is recommended that the proposer contact the Permit Engineer Ken Crosier at the District Two office. 2. The site is over one acre in size, thus a DPW Storm Drain permit is required. The net change in impervious area for the site is a decrease from existing to proposed conditions. This complies with DPW requirements. Before work commences, the proponent should fill out the attached Storm Drain permit application and receive approval from the DPW. 3. The width of the existing driveway is narrow for a two-way vehicle pattem. Given the historical context of this site, caution signs must be placed at both ends of the driveway. "Caution Narrow Drive 2-way Traffic" for entering vehicles and RECEIVEkarrow Drive MAR it b 2006 Yield to PLANNING BO ARD Entering Vehicles" for the exiting vehicles 4one option. This would allow vehicles leaving Main St. to have he right of way for increased safety. These signs-should be yellow with black lettering and made of a reflective material. 4. The proposed staked haybales surrounding the catch basin within the paved area of Main St. is not appropriate. Perhaps placing filter fabric under the catch basin casting would be more feasible. It recommended that a sedimentation barrier (haybales and/or silt fence) be placed at the back of the property in addition to the haybale barrier shown on Main St: The owner will be responsible to ensure that they or the site contractor maintain the sedimentation barriers. If off-site erosion and sedimentation does occur, the owner will be responsible for remediating any negative impacts. 5. The owner or his contractor must notify DPW Engineering at 786-0400 x 275 for inspections as follows: a. Erosion and Sediment Control Inspection -At least 48 hrs notice for inspection of installed erosion and sediment controls. Land disturbing activities may not begin until erosion and sediment controls have been inspected and approved. b. Final inspection - once work is complete, soils are stabilized.
