Per your request dated November 21, 2000 we have reviewed the revised plans for the above
referenced project entitled, "New Addition For: Braman Chemical Enterprises, Inc., 147 Almgren Drive
Agawam, MA" dated 9/18/00, issued 11/21/00 by The O'Leary Co. and we have the following comments:
1. We recommend the following details be included for construction purposes: Sidewalk Section
including Handicap Ramps, Roads/Parking Typical Pavement Section (including gravel base). We
recommend that sidewalks be constructed with 4000 psi concrete.
2. An additional clean-out should be provided approximately 80 feet southwest of the proposed manhole
(at the tee-wye).

W have reviewed the drainage plans and calculations for the above referenced project and we approve
the attached Storm Drain Permit based on the following conditions:
1. Drainage shall be as shown on the plan entitled, "New Addition For: Braman Chemical
Enterprises, Inc., 147 Almgren Drive Agawam, MA" dated 9/18/00, issued 1015100 by The
O'Leary Co.
2_ The applicant should realize that there may be some ponding of water in the parking lot during
the 100 year storm.
In addition, the remaining comments from our memo to the Planning Board dated October 19, 2000
must also be addressed. If you have any questions, please do not hesitate to contact our division.

Per your request dated October 5, 2000 we have reviewed the plans for the above referenced project
entitled, "New Addition For: Braman Chemical Enterprises, Inc., 147 Almgren Drive Agawam, MA" dated .
9/18/00, issued 1015100 by The O'Leary Co. and we have the following comments:
1. The following note should be included on the plans;
"Prior to any installation of any sewer, water or drainage lines,the contractor/owner shall contact the
Town of Agawam's Engineering Division, 786-0400 x 275/8. No piping/appurtenances will be
installed or backfilled without the permission of the division.
If any new water connections are required,the following note should be included on the plans;
"The contractor/owner shall be responsible for testing any existing Town gate valve where a new
connection will be made. The contractor/owner will also be responsible for repairing or replacing such
valve or installing a new valve at the property line in order to allow pressure testing of the new water
main."
2. A Storm Drain Permit will be required from the Department of Public Works. The applicant should
fill out a Storm Drain Permit form and submit it to our division. We have not finished our review
of the storm drain calculations yet.
3. We recommend the following details be included for construction purposes: Sidewalk Section
including Handicap Ramps, Roads/Parking Typical Pavement Section (including gravel base). We

4. The plan should show the location of the dumpster and should show a screening fence with gate
around the dumpster.

6. On Sheet L103 on the southern edge of the parking lot, the spot grade 190.50 should be changed to
191.50.
7. A curb cut permit will be required for the new driveway on Abrams Drive.

Per your request, we have prepared the following punch 1 0 k it ins at the
above referenced subdivision.
1. A Street Acceptance plan must be submitted.
2. The applicant must submit a disk containing the subdivision layout in Town
coordinates.
3. The sanitary sewer must pass a deflection test. Also, a brick shelf and invert must be
built in the manhole in the cul-de-sac.
4. The hydrant in the cul-de-sac must be raised so that the nozzle is between 18 and 24
inches above the ground surface.
5. Since we were not notified of the latest sidewalk construction (along the Barden Street
frontage of house #1231 Main St.), the developer must provide us with the material
slips. Also, there is a cracked sidewalk panel in front of house #24 and two patched
panels in front of house #34 that should be removed and replaced.
6. The driveway apron and adjacent curb on Barden Street for house #1231 Main Street
must be repaired.
7. The applicant must submit a letter from a Land Surveyor registered in Massachusetts
stating that all pins and bounds have been set. If the bound in the metal bound box in
front of lot 6 is in the correct location, then some of the sidewalk was built on private
property.
8. Houses #46 and #56 require two shade trees each. Houses #16 and #24 require one
shade tree each.
9. Regrading and loam and seed is required in the tree belt and behind the sidewalk along
the Barden Street frontage of house #1231 Main Street. Also, the cul-de-sac island
should be loamed and seeded.
10. A final street sweeping and catch basin cleaning must be done.

Description of Project: Proposed construction of a 17,500 sq.ft. addition to an existing
industrial building including a new curb cut and new parking. Braman Chemical is to
