We have recently received information that prevents us from adequately responding to
your request for review and comment on Agawam Golf& Grill Site Plan dated February 16`h
2001. At this time the Massachusetts Department of Environmental Protection (DEP) is.
reviewing a proposed "Additional Gas Monitoring Assessment .Activities" plan that includes
work that must be completed before any proposed on-site construction can begin. We
recommend the planning board suspend presenting our recommendations until more
information becomes available so that complete comments can be made regarding the above
mentioned situation. We also recommend that no approval be granted on the proponents site
plan until he presents a letter from the DEP indicating that it is appropriate to design site
improvements.

Per your request dated February 16, 2001, we have reviewed the proposed building layout .
plan,entitled "Agawam Golf&Grill"369 Main St.Agawam,Ma.dated February 5,2001, for TAM
Sports Enterprises, LLC, prepared by R.C. Designers. This plan review is preliminary in nature
since many of the Agawam Town Code site plan requirements are not addressed. Per direction of
the planning board we are only commenting on the portion of the plan that includes the "Covered
Driving Range"building.
1.) The proposed business requires a parking lot with a recommended 30 minimum spaces. We
also recommend providing a loading and unloading area entirely off the traveled way. We
would like to advise the applicant that he or she will be required to complete an"Application
for Permit to Access State Highway" prior to construction of an entrance or exit to Route
159.
2.) The site plan should show existing and proposed contour lines that extend approximately 30
feet beyond lot lines.
3.) The site plan should show nearby wetlands and the 100-year floodplain boundary'.
4.) We recommend the owner or its representative file a "WPA Form I - Request for
Determination of Applicability" with the Conservation Commission. We also recommend E
showing a silt fence on the site plan to protect areas beyond construction activities.
5.) We recommend that property easements be shown on the site plan.
6.) We recommend Iocating a benchmark on the site plan.
7.) The applicant should obtain a storm drain permit per Agawam's Storm Drain Ordinance.
8.) The applicant should consider the impact from proposed construction on site drainage and
supply calculations as required. The site plan should show any pipes,culverts,or structures,
as required by the above drainage study.
" 9.) Underground and'aooveground utilities as well as service connections to the proposed
building,should be shown on the site plan.
I0.) If a dumpster is proposed,we recommend showing it surrounded by a screening fence on the
site plan.
11.) If a sign or exterior lighting is proposed,we recommend showing it on the site plan.
12.) We recommend showing the following notes on the site plan:
"Prior to any installation of any sewer, water or drainage lines, the contractor/owner shall
contact the Town of Agawam's Engineering Division, 786-0400 x 275/8. No
piping/appurtenances will be installed or backfilled without the permission of the division".
"All plantings must be maintained and that portion of the public way abutting the parcel and
including the treebelt shall be maintained in a safe and attractive condition by the owner of
the parcel."
"The contractor/owner shall be responsible for testing any existing Town gate valve where
a new connection will be made. The contractor/owner will also be responsible for repairing
or replacing such valve or installing a new valve at the property line in order to allow
pressure testing of the new water main."
