Description of Site: On the site there presently exists a single family dwelling. The
applicant must show all structures on the plan and indicate what is to be done with
them.
Provision for Traffic Flow: The plan shows one curb cut on Springfield Street, one on
to Poplar Street and one on to Mill Street. The Mill Street curb cut appears to be one
way. The plan is difficult to read. Widths of the driveways and radii are to be shown.
Parking: No parking is shown.
Drainage: Must be approved by the DPW.
Public Utilities: Al proposed utilities are to be shown on the plan.
Landscaping/Screening and Buffers: The following landscape note must be placed
on the plan: "All plantings must be maintained and that portion of the public way
abutting the parcel and including the treebelt shall be maintained in a safe and attractive

Per your requestdated June 15, 2001, we have reviewed the above referenced site plan entitled
"Site Improvement Plan, JC/JC Realty Trust Car Wash, Agawam, MA" by Joseph D. LaGrasse
Associates, Inc. dated 6/12/01, and we have the following continents;
1. A Storm Drain Permit is required for this site. The applicant must submit drainage<
calculati ns for the 2, 10, and, 100 year storms. Detention will be required to ensure than
the proposed peak rates of runoff will not be greater than:the existing. Also, the plan
should show the existing drainage systems in Springfield, Poplar, d Mill Streets. The=
plan shows the site drainage discharging to an existing catch basin on Poplar Street, but
does not slow what that catch basin is tied into.

10. The sanitary sewer rims, inverts, slopes, size, and type of pipe must be shown. Also, the
plans must show a detail of the proposed sanitary sewer manhole (with brick invert), MDC
trap, and typical sewer trench section. According to John Savioli, Deputy Supt. of Water
& Sewer, the sewer connection shall tie into the existing sewer manhole in the intersection
of Poplar and Mill Streets,
11, The plan must show the proposed water connection location, size, and type of pipe. The
existing �vater mains are,not shown correctly. The applicant should plan on connecting to
the 8" water main in Mill Street at the intersection with Poplar Street Oust no of the
existing drain manhole). See John Savioli if there are any questions.
12. The radius curbing at the Mill Street and Poplar Street entrances shall be vertical granite
curbing. The radius curbing on Springfield Street can be bituminous curbing. Wherever
curbing is shown on the plan, its type should be noted (the information provided is not
clear).
13. The following notes shall be added to the plan:
"Prior to any installation of any sewer, water or drainage lines, the contractor/owner
shall contact the Town of Aga is Engineering Division, 786-0400 ext. 275. No
piping/appurtenances will be installed or backfilled without the permission of the division."
"All plafttings must be maintained and that portion of the public way abutting the parcel

Leveling Pad Preparation
VERSA-LOK walls should be placed on
a leveling pad of well-compacted
gravel, crushed stone or coarse sand
approximately 6" thick and 24" wide.
The first course of VERSA-LOK units
should be embedded below grade
approximately 1/10th of the exposed
wall height. Excavate to lines and
grades you've established for your wall,
allowing for the leveling pad and unit
embedment. Place and compact leveling
