AES, 506 Enterprise Drive, Rocky Hill CT., Dated August 5, 1998, Scale 1" = 20' " and have the following comments: After visiting the subject site on September 30, 1998, the construction work that was done for the previous site plan at this location ( "Bell Atlantic Nynex Mobile, Corner of Meadow Street and Cross Street, Agawam Mass., Site Development Plan Telecommunications Facility, Prepared by URS Greiner, Inc. AES, 500 Enterprise Drive, Rocky Hill CT., Dated June 1997, Revised July 1997, Scale 1" = 20'") has not been completed and some of the work is not done according to the signed and approved plan. 1.) The proposed 26 Arborvitae (8' - 10' mature) that should be planted outside the proposed fence on the north, south, and west sides has not been done. 2.) The proposed 25' x 25' concrete pad that was to be built for the 180' Lattice Tower is approximately 8' x 8'. 3.) The 12' proposed access driveway was not built according to plan, it has been shifted easterly. The gate for the access drive into the enclosed area has been shifted easterly as well. We see no problem with this new location. 4.) The sediment/ control barriers have been taken down. They should remain in place until all work has been completed (26 arborvitae have not been planted). 5.) The parking stall has not been painted and the location of the gate entering the enclosed area is where the proposed parking stall was supposed to be. 6.) The fence was not installed according to plan. It is approximately 55' in width on the northern and southern sides (according to plan 59.26) and it is approximately 90.5' on the eastern and western sides (according to plan 87.22'). The equipment building is approximately 3' outside the fence line on the western side and the plan calls for it to be 2' within the fence line. We have no problem with the position of the fence. 7.) 4n the northwest corner of the fence there is an extra gate to enter the enclosed area to access electric meters. The gate and the electric meters were not shown on the plan. 8.) The pavement repair in Meadow Street Ext. should be redone. The pavement z repair section does not match the existing pavement surface in the public right of way. The pavement repair should comply with the standard that was submitted on the approved plan as well as heating the edge of pavement adjacent to patch with infrared equipment to seal the joints, Applicant should notify the Engineering Department at least 24 hours before the pavement repair takes place. The newly submitted plan does show the following: where the access drive has been built, the location of the entrance gate, the equipment building extending approximately 3' outside the fence line, the correct dimensions of the fence, and an 8' x 8' concrete pad for the 180' lattice tower. It mentions the 26' arborvitae by others (which have not been planted) and it does not show the location of the extra gate on the northwest corner of fence to get access to the electric meters which also are not shown. 9.) The location of the proposed sediment/ erosion control should be shown on the plan and it should encompass all erosion from construction.
