Per your request, we have reviewed the above referenced Form A plan entitled "Revised
d Plan of Lot 19B, Gold Street, Agawam, M,A, owned by Suffield Street Partnership" by
Smith Associates Surveyors, Inc., Scale 1"=80', dated Oct. 1998 and we question
whether the application is appropriate for Form A approval. For example., in order to
create lot 19D, either a subdivision may have to be created, or the applicant possibly
should petition the City to create a public street in the designated 50' right-of-way. We
recommend that this matter be addressed before this plan is approved.
