The following note should be added to the plan, "Prior to any installation of any sewer, water or drainage lines, the contractor/owner shall contact the Town of Agawam's Engineering Division, 786-0400 ext. 275/8. No piping/appurtenances will be installed or backfilled without the permission of the division." 2.) A curb cut permit will be required by the Department of Public Works. 3.) All proposed and existing radii should be labeled (especially the proposed driveway that is entering Garden Street). 4.) An approved Storm Drain Permit was issued for this site on September 9, 1989. Calculations should be submitted for the sizing of the proposed 12" RCP culvert that is proposed for underneath the driveway entering Garden Street. 5.) Additional existing contours should be added to the plan; for example, the drainage swale on the southern and mainly eastern limits of the site to make sure the proposed culvert is at a correct elevation. Proposed contours are required for the site, especially the new driveway entering Garden Street, the culvert underneath the driveway and the changes that will be made to the drainage swale. Rip-rap should be added to the 6" ADS pipe entering the Retention Area on the west side of the property to protect the Retention Area from erosion. The invert of this pipe should be included on the plan. 7.) The proposed 6" berm should be labeled on the plan. A berm should be added east of the parking spaces 15 - 18. 8.) Trees should be labeled "to remain" or "to be removed." There are several large conifers that will have to be removed for the proposed driveway entering Garden Street. 9.) There is an existing culvert entering the drainage swale east of the building (outlet for two catch basins on Garden Street) that is not shown on the plan, its invert should be included as well. 10.) All existing parking spaces should be included on the plan (parking spaces in trucking turn around area are not shown on the plan). The parking spaces shown on the submitted plan are being utilized at this time. This plan does not include any additional or proposed spaces. The number of every vehicle used in connection with the business should be shown on the plan. Show a comprehensive plan of the ultimate parking layout both existing and proposed which should include twenty spaces. 11.) The submitted plan indicates that the truck turn around is gravel, currently it is a paved surface. The paved truck turn around was approved on October 18, 1990 by the Planning Board. If it is proposed to be a gravel truck turn-around, it should be labeled "proposed." 12.) There are four large metal containers north of the truck turn around that are not on the plan and there was two dumpsters in front of these containers. The approved Site Plan for this site, signed by the Planning Board on October 18, 1990, included a 17'x 9' concrete pad with 6' screening fence for the dumpsters north of the future expansion. This should be included on the plans. Any additional containers or dumpsters that are proposed with this addition should be shown on the plan.
