Stone, DPW Supt.: Scott Anderson, Anderson Builders, Planning From: James T. Daley, P. E. Date: November 1, 2000 Subject: Anchor Electric SP327 I met with Scott Anderson today at subject site at 687 Silver Street. Scott advised me that he will lower the detention pond's outlet pipe's upstream end by nine inches ( to elevation 188.56 ), while keeping the downstream end at the present elevation ( 188.38 ) and maintaining a constant straight profile grade, all by November 8, 2000. He will call this office to advise of the exact date this work will take place. I received a letter from Anchor Electric's owner advising me that he did not want the trees planted in the rear of the building as shown on the site plan. Considering and subject to the above, we are signing off on this project. s:1sp1327101.wpd FRo,w THE DESK OF... JANiEs T.DALEY.P.E. Stone DPW Supt., Planning, RSS, File From: Engineering Department Date: June 2, 2000 Subject: Anchor Electric Service - Silver Street- SP 327 - Certificate of Occupancy As per your request of June 1, 2000 we are notifying you as to the status of the Engineering Departments' site inspections for this project. A field inspection was performed June 2, 2000 and we note the following: 1. The 100 foot detention basin at the rear of the site appears to be built incorrectly. Its' location, inverts in and out, and general construction do not agree with the site plan as submitted. A field crew from our division will have to survey this detention basin and the surrounding area to determine if the basin is built according to the plan. 2. There are two additional roof leaders not previously shown. One is approximately 35 feet East of the Northwest corner of the building. The second is approximately 45 feet East of the southwest corner of the building. It is not known where these two roof leaders drain. 3. General fill and grading are required. 4. Logs and debris need to be removed. 5. Landscaping, loaming and seeding, striping/signage are not complete. 6. There is heavy erosion of soil into the Northwest corner of the gabion wall threatening the stability of the nearby condenser pads' foundation. 7. Even though other in-ground utlities have passed inspections and testing, we recommend holding the Certificate of Occupancy until these issues are addressed. Therefore, we cannot recommend that a Storm Drain Permit be issued. Our office was never notified as to any of the above mentioned construction. If we had been we would have certainly made sure that those items were constructed correctly. If you have any questions, please do not hesitate to contact our department. Sincerely, j ames T. The roof drains need to be specified. 2. Curbing reveal must be be 6.5 inches high or match existing curb if located in the Town of Agawams' Right of Way and be included on the detail sheet. 3. Erosion control drawings need to be added to the detail sheet. •f 4. This project will require import fills. A note could be added to the plans stating that these materials meet the Towns' specifications for their respective use (processed gravel, bank run gravel, etc.). The gradation for these various products could also be detailed on the plans. 5. The 2 foot high dimension for the Gabion wall as shown on Page 1 disagrees with the 3 foot high dimension shown on Page 2 and must be clarified. 6. These latest revised plans specify the water main to be 6 inch copper piping. This was probably a typo and we suggest the use of 6 inch Cement Lined Ductile Iron Class 52 water pipe which is more commonly used for this diameter of pipe and is less expensive. 7. We would like to see the entrance drive width increased from 24 feet to 25 feet. 8. The drainage system needs a drain line connecting the two leaching catch basins together at the rear of the building and then a drain line connecting the leaching catch basin at the Northeast corner of the building to the catch basin at the Eastern end of the parking lot. This will act as a secondary relief to the system under extreme storm conditions. 9.
