Subject: Form-A #153 - 131 North West Street - Hastings
Per your request dated April 20, 1999, we have reviewed the plan entitled
"Division of Land in Agawam, MA; For Arthur E. Hastings (Owner); by Alfred A.
Melien, R.L.S., P.E. Dated May 3, 1999," and we have the following comments:
1. There is an inconsistency between the lot lines for 131 North West Street on
the Assessors Map and the Form-A.
2. House number 155 is labeled incorrectly on the assessors map. The correct
location of this house is shown on the Form-A.
3. The house number for the center of the proposed Lot 2 on the Form-A is 143.
4. We do suggest that the zoning of the area is labeled as "Residence A-2" on the
plan.

At its duly called meeting held on May 20, 1999, the Agawam Planning Board voted to deny the
"Approval Not Required Plan" for a parcel of land located on North West Street, plan prepared
by Alfred Melien and dated May 3; 1999. The reason for the denial was the discrepancies
between the plan and the Town's Assessor's plans.
