Hastings (Owner); by Alfred A. Melien, R.L.S., P.E. Dated May 3, 1999," and we have the following comments: 1. There is an inconsistency between the lot lines for 131 North West Street on the Assessors Map and the Form-A. 2. House number 155 is labeled incorrectly on the assessors map. The correct location of this house is shown on the Form-A. 3. The house number for the center of the proposed Lot 2 on the Form-A is 143. 4.
