Per your request dated June 10, 1999 we have reviewed the plan entitled, "Springwich
Cellular Limited Partnership, Wireless Communications Facility, Lennie's Fiberglass, 1804 Main
Street, Agawam, MA, by URS Greiner Woodward Clyde A E-S, 500 Enterprise Drive, Rocky
Hill, CT, dated 5/14/99, scale V = 40'" and we have the following comments:
1.) As a courtesy to the Conservation Commission, we note that the location of the proposed
building is approximately 60' from the flagged wetlands and stream. The proposed
building will be built on an existing gravel area, an additional +/-212 s.f. impervious area
is being created. The plan includes the location of erosion/sedimentation control devices
and details as well.
2.) As per the Storm Drain Ordinance, any site over an acre will require a Storm Drain Permit
from the Department of Public Works. An application is enclosed (We are carbon copying
to URS Greiner).

Description of Site: The location of the proposed antenna and equipment building is
the Omnipoint site at 1804 Main Street. The Planning Board approved the Site Plan for
this tower on February 20, 1998. The Site Plan approval letter is enclosed for your
information. One of the conditions of the approval was the replacement of some of the
trees which had to be removed. Other trees were proposed to be planted along the
northern property line. To date these plantings have not been done.
Provision for Traffic Flow: Unchanged.
Parking: Not shown.
Drainage: Unchanged.
Public Utilities: Not shown.
Landscaping/Screening and Buffers: As indicated above, the landscaping proposed
by Omnipoint has not been installed.

Exterior Lighting: Not shown.
Rendering or Elevations: No elevations have been provided for the new equipment
building. An elevation of the pole with the new antenna has also not been provided.

Other Comments or Concerns: During a earlier review of the installation of a new
antenna at the Bell Atlantic site at the Southend Bridge, Bell Atlantic was ordered to
address deviations from their approved plan. This is a similiar situation.

At its duly called meeting held on January 19, 1998, the Agawam Planning Board voted to
approve the Site Plan entitled "Site Plan Modification, Omnipoint Communications, Inc"
prepared by USR Greiner and dated February 19, 1998 with the following conditions:
1. The Planning Board approval is conditional upon the meeting of all
conditions of the Board of Appeals approval date December 2, 1997.
Since the proposed location of the tower has been changed, a revised
approval of the Board of Appeals may be necessary.
2. All Engineering comments must be addressed. Since the revised plan
was received at the Planning Board's January 19, 1998 meeting,
Engineering review of the plans was not possible prior to Planning Board
action.
3. A detail revised landscaping plan shall be submitted. This plan will be
prepared after input is received from the Director of Planning and the
Planning Board. The plan shall address the area to the east of the tower
location.
Three (3) sets of the revised Site Plan incorporating the above shall be submitted to the
Planning Board for its approval. If you have any questions, please do not hesitate to

Attached please find a memo from Stephen P. Ruzzo of UniSite to Dominic Urbinati, Commissie ie
of Buildings, regarding the planting of trees at the 1804 Main Street site. The planting of trees*vas
a requirement of the Planning Board in its February 20, 1998 approval of the Site Plan fytthe-A--`;
construction of a telecommunications tower. On June 17, 1999, the Planning Board had beare it t F
a petition by SNET to place another antenna on the tower located at 1804 Main Street. PHr to r
action on this petition, the site was visited to ensure conformance with the approved plan_ It was
discovered that the trees that were to be planted still have not been planted even though the Ruzzo
memo of December 14, 1998 indicated that they would be planted by May 1999.
The Board will take no action on the SNET petition until it is satisfied that the conditions of the
approval for the tower have been met. The Board suggest that since this is not the time of year for
planting of trees, particularly large trees, that a bond be posted until the trees can be planted in the
Fall. The Town would be happy to provide you with an amount sufficient to cover the cost of the
trees.
Please be advised that if no action is taken regarding this matter, the Board will request that the
Building Inspector condemn the site. If you have'any questions, please contact the Planning Office
at 786-0400, extension 283.
