are incorrect. There are pieces of varying thickness and curvature. The acceptance plans call for
all vertical granite curbing here to be installed with a uniform thickness and radius. Please note
that a final inspection will be based in part by the approval of these corrections.

Roadway: Garden Street is a heavily traveled roadway for commuter traffic and commercial
traffic that is going into the Industrial Park. The roadway in front of 223 Garden Street is two
lanes wide and approximately 38 feet in total width from curb to curb. The north bound travel
lane is approximately 12 feet wide and has an 8 Moot breakdown lane and a fog line that is 1 '/2
foot from the curb. The south bound travel lane is approximately 12 feet wide and has a 4 '/2 foot
breakdown lane. The on ramp to Route 57 east is a short distance to the north of the property
line of 223 Garden Street,
Visibility: The visibility exiting the proposed driveway at 223 Garden Street is more than 550
feet to the north and 275 feet to the south. Visibility to the south is reduced because of the bend
and hill in Garden Street in the vicinity of the West Springfield Fish & Game Club.
Lighting: There are WMECO light poles to the north and south of the property. There is also a
WMECO light pole directly across the street from 223 Garden Street.
Traffic Controls:There are no traffic controls present.
Sidewalks: There are no sidewalks present on either side of Garden Street in this area.
School Zones: There are no school zones located within 1 '/2 radius.
Playgrounds: There are no playgrounds located within 1 % radius.
Parking. The site plan is in compliance with the Town Ordinance on Handicapped Parking.
Traffic Unit Impact Estimates:None provided.
Other: The driveway has been moved northward from the original plan. This will give more
visibility to the south when exiting the driveway.

3. All Engineering comments are to be met.
In addition, the highway access permit which was recently submitted to the Planning Office to satisfy

permission is hereby granted to Donald Veftnette to enter upon the State Highway Layout in the Town of
Agawvam known as Route 57 or the Garden Street for the purpose of constructing and utilizing a
commercial access on the northerly side of the highway(Lot#223).

considered upon receipt of 1~HWA approval,determination and receipt of an Enhancement Value, if any,
and receipt of a Highway Alteration Plan and airy other conditions necessary to effect the Layout

The rights granted under this permit are subject to the following conditions established by the
Massachusetts Mghway Commission—Item No_4,Right of Way Bureau,dated September 27,2000,
subject to further revision:
l. The Grantee shall be responsible for any Enhancement Value,to be determined by the

requirement. A copy of all correspondence sha:Il be forwarded to the District Office. Permission from other
property owners must be obtained to perform any work within their property,outside the limits of the State
11ghway Layout.
The Grantee and/or subsequent owners are advised that this permit is intended to grant temporary access
and egress for the 5,070 squafm foot Antobedy Facility only. Any further development,expansion,new
business,or change in use shall require the filing of a request for a determination of need for a new permit.
Any expansion,new development or change in use shall be subject to the "Massachusetts Environmental
Policy Act,Chapter 30,Sections 61 and 62a-h".
Said access shall be construe so as to prevent the drainage of water and debris from entering directly
upon the highway.
The access is to,extend from Station 32+51±to Station 32+75±at the State Highway Layout Line, flaring to
extend from Station 32+36±to 32490±at the ea L nng edge of the paved highway.
The outer edge of each access shall be edged with granite curb to the State Highway Layout Line. All of
said curb shall be six(6")inches higher than the grade of the access.
The existing accesses shall be abandoned by removing the bitwminous concrete from within the State
Highway Layout,loaming,liming,seeding the area and replacing the gmnite curb.
The access shall be surfaced with Class 1,Bituminous Concrete,Type I-1 to the limits as shown on the
attached sketch and shall have a six(6")inch foundation of compacted gravel and shall butt and not overlap
the existing edge of the hardened surface_That portion of the access within the State Highway shall be
maintained by the Grantee at their expense and to the satisfaction of the Director.
The area oil each side of the access shall be graded,foamed,fmtilizc ,limed and seeded.
The finished construction shall conform the attached plan,prepared by Durkee,Write,Towne&
Chapdelaine,Civil Engineers&Land Surveyors,entitled,"Work Sheet,Agawam,Massachusetts,
Vermette Auto Body,Tnc.,Amended Curb Cut,"dated May 5,2000,with the following exception:
within the limits of the State Highway Layout,granite etab shall be installed in place of cape cod bcam.
At the end of each workday,the Grantee shall keep the highway surface free of any dirt,dust or debris of

Refleetorized plastic drums shall be used in the highway work zones;the use of mew drums is not allowed.
The Grantee's property shall be graded in such a manner that storm water or other drainage shall not flow
directly onto the State Highway Layout.
Any ponding of water,which may occur in this area as a result of the work herein described,shall be the
responsibility of the Grantee..
The existing gutter grade and line shall not changed. The highway surface shall not be dis�.

No parking is allowed within the limits of the State Highway layout.

shall be charged to the Grantee.
All work proposed and materials used shall conform to the"Massachusetts Highway Department
Standard Specification for Bighways and Bridges," 1995 Metric Edition, and the'°Massaebrsetts
Highway Department Construction and Traffic Standard Details," 1996 Metric Edition,as amended.

Subject to all the terms,conditions and restrictions printed or written below,and on the reverse side hereof,
permission is hereby granted to Donald Vermette to enter upon the State Highway Layout in the Town of
Agawam known as Route 57 or the Garden Street for the purpose of constructing and utilizing a
commercial access on the northerly side of the highway{Lot#223).
The rights granted under this permit are issued for temporary access only. Permanent access rights will be
considered upon receipt of FHWA approval,determination and receipt of an Enhancement Value, if any,
and receipt of a Highway Alteration Plan and any other conditions necessary to effect the Layout
designation change.
The rights granted under this permit are subject to the following conditions established by the
Massachusetts Highway Commission—Item No.4,Right of Way Bureau,dated September 27,2000,
subject to further revision:
l. The Grantee shall be responsible for any Enhancement Value,to be determined by the
Mam ighway Departments Right of Way Bureau,for any increase in value of the property as a
result of the approval for the layout designation change.
2. As directed,the Grantee shall be responsible for removing and resetting MassHighway Bounds to
effect the highway alteration.
3. Any expansion or changes of use to the property are subject to the approval of the Department and
the Massachusetts Environmental Policy Act.
4. The existing 2 residential accesses shall be eliminated.
5. A single commercial access shall be Iodated nearest the southerly most property line_
The rights granted under this permit are subject to approval by the Federal Highway Administration
(FHWA).
As directed,permission is granted to remove and reset the Massachusetts Highway Bound in accordance
with the following. Said bound(s)shall be located by a Registered Civil Engineer or Land Surveyor. A
sketch of the measurements taken shall be submitted to the District Director designated below. The bound
shall be reset according to the measurements taken. All expense shall be borne by the Grantee.
At the Grantee's expense, a Highway Alteration Plan prepared in accordance with Department specifications
in which the above stated layout changes are recorded legally.
The Grantee shall be responsible for obtaining and then maintaining sight distance on the easterly side of
the proposed access. Permission is granted to mow and then maintain that portion of the State Highway

Layout from the proposed access,easterly to the end of State Highway Layout at Station 30+77.24. The
Grantee shall notify the abutting property owner,in writing,of this requirement. The abutting property
owner shall be given 10 working days to notify the District of any concerns or comments related to this
requirement. A copy of all correspondence shall be forwarded to the District Office. Permission from other
property owners must be obtained to perform any work within their property,outside the limits of the State
Highway Layout.
The Grantee and/or subsequent owners are advised that this permit is intended to meant temporary access
and egress for the 5,070 square foot Autobadv Facility only. Any further development,expansion,new
business,or change in use shall require the filing of a request for a determination of need for a new permit.
Any expansion, new development or change in use shall be subject to the"Massachusetts Environmental
Policy Act,Chapter 30,Sections 61 and 62a-h".
Said access shall be constructed so as to prevent the drainage of water and debris from entering directly
upon the highway.
The access is to extend from Station 32+51±to Station 32+75±at the State Highway Layout Line,flaring to
extend from Station 32+36f to 32+90t at the existing edge of the paved highway.
The outer edge of each access shall be edged with granite curb to the State Highway Layout Line. All of
said curb shall be six(6'�inches higher than the grade of the access.
The existing accesses shall be abandoned by removing the bituminous concrete from within the State
Highway Layout,loaming,liming,seeding the area and replacing the granite curb.
The access shall be surfaced with CIass 1,Bituminous Concrete,Type I-1 to the limits as shown on the
attached sketch and shall have a six(6")inch foundation of compacted gavel and shall butt and not overlap
the existing edge of the hardened surface_That portion of the access within the State Highway shall be
maintained by the Grantee at their expense and to the satisfaction of the Director.
The area ou each side of the access shall be graded,loamcd,fertilized,limed and seeded.
The finished construction shall waform the attached plats,prepared by Durkee,White,Towne&
Chapdelaine, Civil Engineers&Land Surveyors,entitled,"Work Sheet,Agawam,Massachusetts,
Vennette Auto Body,Inc.,Amended Curb Cut,"dated May 5,2000,with the following exception:
within the limits of the State Highway Layout, granite curb shall be installed in place of cape cod berm.
At the end of each workday,the Grantee shall keep the highway surface free of any dirt,dust or debris of
any nature,resulting from the work_ 71 a Grantee shall utilize mechanical sweepers,water,calcium
chloride or bitumen and sand,in accordance of Section 440,Roadway Dust Control,of the 1995
MassHighway Department's Standard Specifications for Highways and Bridges,as amended.

Per your request dated March 1, 2000 we have reviewed the site plan entitled; "Site, Plan,
Agawam, Mass., Prepared For Vermette Auto Body Inc., 787 Silver Street Agawam, Mass." by
Durkee, White, Towne, & Chapdelaine dated February 2, 2000 scale 1" = 20'. An inspection of
the subject site by the Engineering Department tools place on March 13, 2000 to review the data
on the site plan and we have the following comments:
1. A storm drain permit will be required and drainage calculations must be submitted before
final approval of a site plan.
2. Existing contours should be carried throughout the site to show all pre-construction
grading. Clarify which contours are existing.
3. If the intent of new grading is to create a swale along the Eastern edge of the property and
proposed to drain Northerly, then it should be noted as such. Permission from the
MCMahons will be needed.
4. A positive outlet plus an overflow device with details are needed for the retention pond at
the West end of the parking lot.
5. Radii and curbing need to be specified where the proposed driveway joins Garden Street.
6. The plan should state what type of curbing and construction is proposed at the existing
driveway closing.
7, Place yield sign on private property facing drive exiting traffic.
8. The number of employees & customers should be listed on this site plan to allow
Engineering to determine the quantity of appropriate stalls.

9. Parking layout data is missing (parking stall dimensions, driveway width, travel direction
arrows, etc.).
10. Existing/proposed utilities information must be defined and included on the plan (pipe
diameters, pipe compositions, inverts, etc.).
11. A handicap ramp note with elevations should be drawn for the office entrance doorway.
12. Provide construction details as per these comments and also for sewers and water mains
as needed.
13. The site requires a large amount of import fill material. The plan should list the different
types/gradation of fill proposed and where placed.
14. It is unknown whether the pole detailed at the Southeast corner of the lot (at the East side
entrance drive radius) is intended for lighting purposes. In general it is recommended that
lighting be installed on site to assist vehicle access and parking within the complex.
15. The following notes will be added;
a.) "Prior to any installation of any sewer, water or drainage lines, the contractor/owner
shall contact the Town of Agawam's Engineering Division, 786-0400 ext.275/8. No
piping/appurtenances will be installed or backfilled without the permission of the division".
b.) "All plantings must be maintained and that portion of the public way abutting the
parcel and including the tree belt shall be maintained in a safe and attractive condition by
the owner of the parcel".

Roadway: Garden Street is a heavily traveled roadway for commuter traffic and commercial
traffic that is going into the Industrial Park. The roadway in front of 223 Garden Street is two
lanes wide and approximately 38 feet in total width from curb to curb. The north bound travel
lane is approximately 12 feet wide and has an 8 '/2 foot breakdown lane and a fog line that is I %z
foot from the curb. The south bound travel lane is approximately 12 feet wide and has a 4 '/2 foot
breakdown lane. The on ramp to Route 57 east is a short distance to the north of the property
line of 223 Garden Street.
Visibility: The visibility exiting the proposed driveway at 223 Garden Street is more than 550
feet to the north and 220 feet to the south. Visibility to the south is reduced because of the bend
and hill in Garden Street in the vicinity of the West Springfield Fish& Game Club.
Lighting: There are WMECO light poles to the north and south of the property. There is also a
WMECO fight pole directly across the street from 223 Garden Street.
Traffic Controls: There are no traffic controls present.
Sidewalks: There are no sidewalks present on either side of Garden Street in this area.
School Zones: There are no school zones located within 1 %Z radius.
Playgrounds: There are no playgrounds located within l % radius.
Parking:The site plan is in compliance with the Town Ordinance on Handicapped Parking.
Traffic Unit Impact Estimates:None provided.
Other: It would be my recommendation that the driveway be relocated 25 to 30 feet more to the

2. Existing contours should be carried throughout the site to show all pre-construction
grading. Clarify which contours are existing.
3. If the intent of new grading is to create a Swale along the Eastern edge of the property and
proposed to drain Northerly, then it should be noted as such. Permission from the
McMahons will be needed.
4. A positive outlet plus an overflow device with details are needed for the retention pond at
the West end of the parking lot.
5. Radii and curbing need to be specified where the proposed driveway joins Garden Street.
6. The plan should state what type of curbing and construction is proposed at the existing
driveway closing.
7. PIace yield sign on private property facing drive exiting traffic.
8. The number of employees & customers should be listed on this site plan to allow
Engineering to determine the quantity of appropriate stalls.

9. Parking layout data is missing (parking stall dimensions, driveway width, travel direction
arrows, etc.).
10. Existing/proposed utilities information must be defined and included on the plan (pipe
diameters, pipe compositions, inverts, etc.).
11. A handicap ramp note with elevations should be drawn for the office entrance doorway.
12. Provide construction details as per these comments and also for sewers and water mains
as needed.
13. The site requires a large amount of import fill material. The plan should list the different
types/gradation of fill proposed and where placed.
14. It is unknown whether the pole detailed at the Southeast corner of the lot (at the East side
entrance drive radius) is intended for lighting purposes. In general it is recommended that
lighting be installed on site to assist vehicle access and parking within the complex.
15. The following notes will be added;
a.) "Prior to any installation of any sewer, water or drainage lines, the contractor/owner
shall contact the Town of Agawam's Engineering Division, 786-0400 ext.275/8. No
piping/appurtenances will be installed or backfilled without the permission of the division".
b.) "All plantings must be maintained and that portion of the public way abutting the
parcel and including the tree belt shall be maintained in a safe and attractive condition by
the owner of the parcel".
