Subject: ' SP - 432 - MicroTest- Punch List
On Monday July 18, 2005, 2 members from Engineering visited the subject site to perform a
walk through inspection with Randy Campbell, the project superintendent for the subject site. There are
no significant deficiencies for utilities or site work. Engineering approves the release of the Certificate
of Occupancy.
It is recommended that the following items be addressed before the issuance of the Certificate of
Compliance by the Conservation Commission. These items pertain the wetland Order of Conditions.
Please see the attached list describing the completion of these items.
The trash racks for the outlet structures in each detention pond still have to be installed. They are
shown in the detention pond details. The installation of these racks should not affect Mr. Campbell's
work schedule ("completed week of 7/I 8/05"). The subject site has recently been hydro-seeded. The
establishment of vegetation+/or erosion repairs must occur at both detention ponds as required.
As per Mr. CampbelI's work schedule:
I. Water quality swale in northern detention pond has to be re-constructed as per plan.
2. Grout outlet pipe in south detention pond's outlet structure.
3. Mr. Campbell to submit letter certifying correct MA wetland seed mix used in detention ponds.
This mix to be re-applied again once north detention pond re-constructed.
4. Add stone and re-shape all rip-rap splash pads as per details.
5. In CB 2, hood not installed. Grout 6" inlet pipe.
1 6. The new hydrant needs a 6" extension.
Once again,the above listed items must be addressed before issuance of the Conservation
Committee's Certificate of Compliance.

approval to issue a revenue bond on behalf of MicroTest Laboratories, Inc./MicroTest

COMPANY: MicroTest Laboratories, Inc. (MTL) was founded in 1983 by
Dr. Steven Richter as a contract microbiological testing
laboratory. MTL has expanded its business to include a
comprehensive array of analytical and chemical testing services
as well as pharmaceutical formulation fill finish drug
manufacturing. This process delivers purified bulk drugs to
MTL where they are appropriately formulated and aseptically
filled into smaller vials.
MicroTest is an ISO certified laboratory that meets both United
States Pharmacopeia and FDA requirements. Clients include
Abiomed; Johnson & Johnson; C.R. Bard; Boston Scientific;
Tambrands; Alcon Lays; Allergan; Genetic Institute; Genzyme;
Biogen; Immunex and many other prominent pharmaceutical,
biotech and medical devise manufacturers.
MicroTest Properties, LLC owns the real estate and leases the
building to MicroTest Laboratories, Inc. The companies have
identical ownership structures.

The following comments were discussed with the site contractor Mr. Joe Frenette:
1. On the week of April 26't', 2004, the Department of Public Works had tested the existing 6" sewer
lateral from the existing sewer manhole at the end of the street located in the cul-de-sac. The purpose
was to determine the integrity of the pipe and the feasibility of using the existing sewer inverts already
established in the street. The DPW's field investigation found 58 feet of pipe located in the direction
of the proposed entrance/exit. Further site information such as the invert elevation at the end of the
58 feet would have to be located in the field and decided upon by the owner, engineer, and contractor
on using this existing sewer lateral. If the owner is planning on using this existing sewer lateral please
resubmit the site plan with any information pertaining these alterations to establish an accurate
depiction of the proposed work.
2. The DPW also found the existence of a 6" water lateral west of the existing sewer manhole in the
cul-de-sac that could become an option, if all engineering factors are considered, instead of the
proposed 8" water tap. If this change is made, please resubmit the site plan with any information
pertaining these alterations to establish an accurate depiction of the proposed work.
3. We approve of this revised site plan dated April 2, 2004.

At its duly called meeting held on January 29, 2004,the Agawam Planning Board voted to approve
the Site Plan entitled"Microtest Labs Inc. Facility Expansion, Microtest Labs,Inc., 104 Gold Street,
Agawam, Massachusetts" prepared by Baystate Environmental Consultants, Inc. and dated
December 2003 and revised January 26, 2004 with the following conditions:
1 The type,height and wattage ofthe exterior lighting to be used is to be submitted to
the Planning Board for their approval.
2. The dumpster location is to be shown on the plan specifying that the dumpster is to
be screened, fenced and gated.
3. The following landscape note is to be provided on the plan: "All plantings must be
maintained and that portion of the public way abutting the parcel and.including the
treebeit shall be maintained in a safe and attractive condition by the owner of the
parcel."
4. The Engineering comments contained in the Engineering memo dated January 29,
2004 and attached must be addressed. j

1. We request one benchmark be shown on the plan.
2. With the expansion of the parking lot and the new proposed access road to the cul-de-sac we
recommend adding some type of travel direction pavement markings and/or signs within the parking
lot.
3. We recommend a minimum of 3 to 1 slopes wherever there is proposed re-grading. Since the two ,
detention ponds have 2 to 1 slopes, and the drainage calculations have been approved slope
stabilization measures should be implemented as needed.
4. Please show the type of curb on the plan to be used on the radius near the cul-de-sac for the curb
cut portion. We recommend using a Bituminous Concrete Berm Type-II (9" wide by 6 1/2" reveal)
to.match the existing.
5. A minimum of 5 feet in depth for all water service piping should be shown on the plan.
6. The following notes should be added to the plans:
"Prior to any installation of any sewer, water or drainage lines, the contractor/owner shall contact
the Town of Agawam's Engineering Division, 786-0400 x 275/8.. No pip ing/appurteriarices will be
installed or backfilled without the permission of the division."
"The contractor/owner shall be responsible for testing any existing Town gate valve where a new
connection will be made. The contractor/owner will also be responsible for repairing or replacing
such.valve or installing a new valve at the property line in order-to allow pressure testing of the new
water main."
7. We have certain standard details we use within the Town. The following details will be sent over
to you digitally in AutoCAD format for your use to include in the detail sheet: Sanitary Sewer
Manhole, Concrete Collar, Sanitary Sewer Manhole Frame and Cover, Hydrant Detail, Building

recommend adding some type of travel direction pavement markings and/or signs within the parking
lot.
3. We recommend a minimum of 3 to 1 slopes wherever there is proposed re-grading. Since the two
detention ponds have 2 to 1 slopes, and the drainage calculations have been approved slope
stabilization measures should be implemented as needed.
4. Please show the type of curb on the plan to be used on the radius near the cul-de-sac for the curb
cut portion. •We recommend using a Bituminous Concrete Berm Type-11 (9" wide by 6 1/2" reveal)
to match the existing.
5. A minimum of 5 feet in depth for all water service piping should be shown on the plan.
6. The following notes should be added to the plans:
"Prior to any installation of any sewer, water or drainage lines, the contractor/owner shall contact
the Town of Agawam's Engineering Division, 786-0400 x 275/8. No piping/appurtenahces will be
installed or backfilled without the permission of the division."
"The contractor/owner shall be responsible for testing any existing Town gate valve where a new
connection will be made. The contractor/owner will also be responsible for repairing or replacing
such valve or installing a new valve at the property line in order to allow pressure testing of the new
water main."
7. We have certain standard details we use within the Town. The following details will be sent over
to you digitally in AutoCAD format for your use to include in the detail sheet: Sanitary Sewer
Manhole, Concrete Collar, Sanitary Sewer Manhole Frame and Cover, Hydrant Detail, Building
Sewer Connection, Permanent Pavement Replacement Detail, Wheel Chair Ramp (bounds not
needed), and Thrust Restraints Field Lok Gaskets. Please add a Bituminous Concrete Berm Type-H
detail since our version is only included in the'Typical Roadway Section.
8. The following permits are needed and can be picked up at the Department of Public Works:
Storm Drain Permit, Road Opening Permit, and a Curb Cut Permit.
OV9. A Sewer Extension Permit may be required depending on the design flow of the expansion. Please
consult the Department of Environmental Protection regulations. If it is required, construction on the
sanitary sewer connection may not begin until the D.E.P. approval is received.
