Associates, 1130 Williams Street,Longmeadow, MA, 01106; Scale: 1"=40; Date: Dec., 2003, and
we note the following:
i. Survey data for the east radius at the entrance to Mark Drive should be added.
2. Zoning lines should he included on the plan.
3. If single family dwellings are to be built on Mark Drive,present street address numbering for
multiple units/lot as shown on this Form A will have to be altered.
