Based on the resubmission of the site plan the Engineering Department received on May 6, 2004, we have the following comments: A. The Department of Public Works policy states that each building needs it's own water service, therefore no access will be allowed to the Berkshire Power Company's existing 12" water main. The proposed 1" copper water service will need to tie into the Town of Agawam's water main in the street. Please file all necessary permits, such as a road opening permit, with the DPW office. B. The Massachusetts Office on Disability(MOD) in conjunction with the Americans with Disabilities Act(ADA), states that for every 1 to 14 parking spaces at least one handicap parking space is required, preferably dimensions to accommodate a van. The plan should show the signage and type of pavement markings for the required handicap space. C. The following note does not have to be included on the plan since the water service is only a 1" copper line. "Prior to any installation of any sewer, water or drainage lines,the contractor/owner shall contact the Town of Agawam's Engineering Division; 786-0400 x 275/8. No piping/appurtenances will be installed or backfilled without the permission of the division". D. Please note the address of the site plan location within or near the title box. Show the existing water line starting in Silver Street extending to the proposed section, also please label the size and type of pipe for both existing and proposed water lines. B. Show the existing sewer line in Silver Street, along with any structures that may impede the path of any sewer pipe, also please label the size and type of pipe for existing and proposed sewer lines. C. Please clearly delineate the difference between any existing pavement areas and proposed pavement areas. Show the dimensions of a proposed parking spaces, and the type of pavement markings.
